@MensahPT Plus ectopic impulses can’t explain the reports of patients who suffer with radicular symptoms. Most common bothersome symptom is often a dull ache, not sharp lancinating pain of an ectopic impulses.

If radicular pain is “a gain of function” and Radiculopathy is a “loss of function” how do we explain painful radiculopathies ie radiculopathy+radicular pain, which is extremely common?
This loss & gain of function is not exactly the truth. Nocicpetion is not a functional gain.

@delellis_marc The Latter. It’s the difference between learning the system in a linear fashion, applying the principles to assess and treat. I think this is definitely under emphasized. I French show an in clinic demo on my courses v a linear dogmatic process.

@DrSethPT @marklaslett_NZ Simple answer, retraction is completely different loading on the cervical joints than extension in sitting. It is nothing to do with shear or range (see Penning next tweet) It is not a new bowel stimulus either, as it is a functional movement use often used in sport.

@MensahPT Sensitisation and ectopic impulses are not a function gain they are normal responses of injured nerves indicating pathologic changes. “ function gain” implies no pathology “abnormal function” would be more accurate and appropriate to indicate pathology.

OMG todays offering from the Poulter PowerPoint archives is this 2006 presentation on differentiating shoulder from neck. Remember this predates the McKenzie EXPOSS study by some 12 years but I did teach most of the people who did the study. 😂 enjoy a blast from the past.
