@ClareGerada @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk Poor workforce planning has been a theme of the NHS for as long as I can remember. I suspect because it is not a solvable problem. @alisonleary1

Question to everyone with PA’s working in A&E.
Has your department implemented the Leng review?
Are PAs still seeing undifferentiated patients with complex conditions? Are they still in Resus?
Remember they must ONLY see minor alignment.
No chest pain, no abdominal pain, no sepsis, no CVS, no IECOPD, no NOFs

@Megsenmumdr Of course I know. I am
Not arguing it’s a bad thing. Just responding to a previous comment about why we are seeing a so called surplus of GPs. On paper we do. But we are still short of GPs as
We don’t have the resources to employ more. But blaming PA’s for this is unfair
English

@ClareGerada U very well know why this is!
Until 3-4yrs ago,GP training places were under filled by UKGs-IMGs typically go into specialties/areas where UKGs dont want to go
It seems u r saying having more IMGs in GP is y u support NOT medically qualified PAs working unsafely as GP substitute
English

@ClareGerada here spreading False Narrative against “overseas trained Drs” (IMGs) to a member of public.
IMGs cannot directly work in GP unless they do GP training & exams & achieve CCT in GP
PAs have needed no medical training, no post grad training to work as GP substitute 🙄

@Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk Doctor unemployment is due to increased recruitment drive for overseas trained doctors coupled with increased supply from expansion of medical school places. Poor workforce planning. How many PA’s do you think there are in UK workforce ‘taking doctor jobs?’
English

@Megsenmumdr @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk Because the funding is going for hospitals. PC receives <9% for >80% activity. Until we rebalance this we continue to fight amongst ourselves and blame others. I have been arguing this for 25 years. We get more of the same.
English

@ClareGerada @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk If we have“Massive shortage of GPs” Y aren’t we funding Practices to employ GPs?
Y divert core funding to ARRS to encourage PCNs to employ PAs to see undifferentiated pts?
U have been co-chair of NHS Assembly advising Govt on Workforce-Y wasn’t retaining GPs prioritised?

@Megsenmumdr @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk Since 2018, the number of GP trainees from outside the UK/EEA increased by 3,568, compared to a rise of 1,674 UK‑trained trainees, a 519% increase versus 46%, respectively.
English

@Megsenmumdr @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk IMG representation in GP training rose from 34% in 2019 to 52% in 2023, the highest among all specialties. From 2018 to 2023, IMGs filled 46% of GP training roles by the end of the period.
English

@donalmurray2002 @ItsnotrightUK It’s picking on one group to the exclusion of others. There are more pharmacists working in GP (and right so) than PA’s.
English

@ClareGerada @ItsnotrightUK I cannot see how saying they need to work to scope is bullying. They are not replacement doctors!!
English

@Jean_Jeannie1 @Mary_Tom11 @ItsnotrightUK @wesstreeting @DHSCgovuk We don’t have enough GPs/ generalists in hospitals / community physicians.
English

@DrJoFranklin @Gt77745Rahul @ItsnotrightUK Of course as with all other practices and hospitals we use skill mix. Why ever not. Have been doing so
Since I started in GP 49 years ago
English

@Gt77745Rahul @ClareGerada @ItsnotrightUK IIRC @ClareGerada confirmed some months ago that Hurley Group did not employ PAs, at least at that time, though I'm quite sure they use every scrap of ARRS funding for other roles.
Have the practice ever used PAs?
If so, why did they stop using PAs I wonder?
English

@ClareGerada @ItsnotrightUK @wesstreeting @DHSCgovuk Doctor unemployment also largely due to dr sub by PAs/AAs/ACPs et al and clearly seen in 10 year plan. PA headcount c 4000plus (no clear data on this) . 4000 training roles and jobs taken from drs who are more productive, safer for patients and cheaper

@ClareGerada @ItsnotrightUK Leng Review recommendations accepted in full by @wesstreeting @DHSCgovuk . It is not bullying to ask if trusts/PCNs/practices are following govt instructions. @ClareGerada are you going to apologise to drs - unemployed/emigrating etc owing to your pursuit of dr subs?

@davidedgarwolf ‘why are doctors so unhappy ?’ I can tell you in 3000 words

A-level results day is always a good reminder — your achievements at 17 or 18 do not define your life.
I flunked mine (D, E, E) for a variety of reasons, yet still went on to build a productive and successful medical career. It cost me more in time and money than I’d like to admit, but where there’s a will, there’s a way.
Don’t let three letters dictate your future.

@LarcombePeter @DrSharandeep @drmattuk @ExplosiveEnema2 @Anisocyte So now using AI is also a no-go area? Sad it’s come to this.

@DrSharandeep @drmattuk @ExplosiveEnema2 @ClareGerada @Anisocyte Every day is a school day, it seems

@ClareGerada @Anisocyte Timestamp 19min 30s
Clare, you state your hope is PAs:
- get independent prescribing
- develop specialist interests
- should be able to get RCGP membership
English

@LarcombePeter @drmattuk @ExplosiveEnema2 @Anisocyte Nothing wrong with getting AI to check posts to make sure accurate ?
