I haven’t seen any of them believing they are equivalent to a doctor. The ones I have seen are clear they work within proper constraints on differentiated patients within their level of competence. I haven’t seen any of them claiming to be doctors, cosplaying as doctors, or otherwise misleading patients. If that does happen then action should be taken.

I love to see PAs publicly try to defend the indefensible. There’s some high level dangerous brainwashing going on in their uni courses and continued by their supervisors to make them believe that they’re equivalent to Drs. #nhs #askforadoctor

The Leng review was a long overdue look into the safety of new roles that should have occurred prior to their role out. Sadly it took deaths and patient harm before it happened. @jamesmurray_ldn must now uphold the review and ensure that PAs cannot see undifferentiated patients

One year after the Leng Review, the High Court has dismissed a legal challenge to its implementation on all grounds.  With patient safety at stake, the Government and the NHS must now act quickly to stop PAs from seeing undifferentiated patients. And had no regulator ensuring their practice is safe over that time The F2 is accredited, standardised and regulated You'll be safer with the F2

PA studies are *unaccredited*. This is a massive deal that has been swept under the rug Accredited courses have external bodies who come in to audit & review the content being taught to ensure it is of a high standard PA studies do not have this external audit process… 🤯 1/3

Physician Assistants (PAs) are NOT legally authorized to independently order these under the Ionising Radiation (Medical Exposure) Regulations (IR(ME)R). (H/T @medicalmodelbri for this finding) The absolute clownery workforce has descended into-with scant regards for the public- while the Royal Colleges sit back and the regulator pontificates? This is how my day has gone. 178 via e-consult with a significant number of things inappropriate for GP to deal with i.e. hospital related work dumps. I see a tonne of consultants here saying “insist” on a Gp appt, fine but where and with whom? General practice is drowning.

@DrSteveTaylor I get the issue around who sees who Steve but surely contract is a major issue im on 141 patients today as duty doc etriaging, my next routine prebookable with a GP is 7th august. Im fitting in as extras today as many as i can and signposting and overbooking

@sophia_edw28123 44% of all appointments in April 2026 were carried out by a GP, nurses 18%, nonGP clinical staff about 25%, then the rest are vaccine clinics, care coorindators. Most people do see GPs, and Gps want to see more but the problem is nobody listening to GPs as to what issues are

@Keir_Starmer Oh just fuck off will you? I've had 7 appointments at my GPs surgery and not actually seen a doctor! 2 nurses ,2 paramedics, 2 physician associates (now, allegedly assistants but not at my surgery) and 1 physician associate Student, for God's sake. You've fkd it for the future.

So all the GP surgeries, ED depts, CDs using PAs as SDMs or to see undifferentiated patients should take note: GMC doesn’t explicitly assess these core capabilities, or even endorse PAs to be used this way. It isn’t taking responsibility for what happens, YOU are. 6/

Then it seemed they’d basically run it through AI rather than read it themselves and largely misunderstood the document. Few days later, rapid U-turn on supporting Leng.

We shouldn’t forget the role of Anaesthetists United and Mr and Mrs Chesterton in bringing the whole issue of patient safety to the fore in the PA/AA debate. The Leng Review was referenced many times in their JR. I hope that they feel at least in part vindicated today

They have significantly more formal, nationally standardised training than we do and yearly cpd cannot change that. Only doing medicine + FY1/2 + GP training can.
