New in AfJEM!
A systematic review & meta-analysis finds no evidence that noradrenaline is superior to adrenaline in managing septic shock.
Big implications for emergency care in resource-limited settings.

The little "chit chats" we have at the start of a meeting or Teams call (where we talk about our weekends, the weather, our pets or the traffic) are really important. Research shows that they actually work as a neural 'warm-up' for a deeper connection.
So the next time we get irritated by the small talk at the start of a meeting & push to start discussion on the topic at hand, we might remember that:
- small talk isn’t filler; it’s an entry point to foster group psychological safety.
- rapport isn’t a ‘nice to have’; it enables the trust needed for meaningful dialogue.
- alignment doesn’t begin with tasks; it begins with shared attention & mutual recognition.
Those first few minutes of “non-essential” chatter are doing critical work. They're building safety, alignment & a shared language that's the foundation for us to work more effectively together: linkedin.com/posts/nick-wri…. By Nick Wright. The stage of maturity of your learning/improvement network or community of practice impacts how much difference it can make. @SFU Faculty of Science identifies three stages:
1) "Knowledge Weaving" prioritises the exchange of tacit knowledge within the network or community. Collaboration & connection enable valuable experiential knowledge to be exchanged.
2) "Collaborative Learning" is about taking that tacit knowledge further to support members to experiment & learn, generating new & emergent approaches which are further shared.
3) "Systems of influence" extend influence beyond the community or network by advocating & supporting systemic change in response to complex challenges.
Many networks/CoPs in health & care never get beyond stage one, when their potential is so much greater.

A new article on the "messy middle" of change. This is the gap in the change process between the intent/ambition for change & the ability of a system, organisation or team to make progress. The messy middle is the hard work of delivering real change. Unless we have methods to motivate people to work towards something new, we may never get past it.
benholliday.com/2025/01/28/the… By @BenHolliday.
It builds on "Kanter's Law" that says that the middle is the hardest part of change: hbr.org/2009/08/change…
@tashwillcocks compares the messy middle to the "groan zone" in the diamond of participation. Getting through it is a relational, not just technical process: linkedin.com/feed/update/ur…
