Our learnings from this translational project: 1️⃣ - Focal monopolar #PFA directly on the myocardium 👉🏻 myocardial lesions with a depth of ~6-8mm 2️⃣ - Focal monopolar #PFA from within the GCV 👉🏻 myocardial lesions with a depth of ~4mm 3️⃣ - after prophylactic nitroglycerin 👉🏻 NO spasm when PFA was delivered >5mm away from the coronary artery 👉🏻 transient spasm without changes in OCT, when PFA was delivered directly on the coronary artery Therefore: Sequential mapping-guided focal PFA from endocardial ventricular outflow tracts and from within the GCV is feasible with a favourable procedural safety profile for the treatment of epicardial PVC.
