LV septal capture is not sufficient for CRT. The main criteria for LBBaP in HeartSync required output dependent changes in QRS and/or LVAT: the sine qua non of conduction system capture. PhysioSync used and reported V1r' and LVAT predominantly- which is likely what the majority are doing.

Our state-of-the-art review on pulmonary HTN due to left heart ds (group 2) now publ in JACC. Poor prognosis, esp when pre-capillary PH develops (PVR >2 WU - CpcPH). No specific Rx exist. We're starting the PULSE-LHD trial of PA denervation in 500 pts with group 2 PH and CpcPH.
