In the past few years, my ICU has prioritized mobility and is tracking our progress. We use the JHHLM score to track what patient have done and to make goals for what they can do. I made drawings like this to help depict each JHHLM score. #MedTwitter #Mobility #CriticalCare

My ICU team has started working on a monthly education series with our providers and nurses. This will be our first month doing it and the topic is DKA. Let me know if you have any DKA management pearls I can share! #MedTwitter #CriticalCare #ICU #DKA #Diabetes

@PASeekingcafien I would contact the Doctor and tell them you are still waiting on the recruiter. We had a NP who we really wanted but decided go with another hospital because our recruiter was taking too long to get back with them.

So say recruiter just doesn’t reply to text or email…. What do I do next? Do I contact the Dr who I’ve also texted/emailed with throughout this process? If they don’t have an answer yet, okay I understand, but can you at least tell me that?

@imTKuknowme @chungk1031 @ChristosArgyrop Actual algorithm is something like: - reason to be sedated? (safety, misery, or RN time) -> prescribe sedative - problems interfering w sleep? (labs, tv, lights, diuretics, noise) -> fix those - can I talk them (pt/Rn) out pharma in 30s? -rx, & back to more pressing issues

I'm a 4th gen doctor. This meant I went to good schools, socialised with other doctor's kids- many of whom also wanted to become doctors and shared tips and resources about medical school entry- and already knew many lecturers when I got in. It's *not* a level playing field. 1/2

You’re 24x more likely to become a doctor if your parents were doctors, compared to those whose parents did any other type of work. Similarly, you’re 17x more likely to become a lawyer if your parents are lawyers. Here’s how it varies among the elite professions.

Allan Walkey on atrial fibrillation in ICU (#SCCM2023) Arrigo 2015: cardioversion for secondary A-fib in ICU rarely works (43% remains sinus at 1hr, 23% at 24hr). Diltiazem was more effective at rate control than amio or digoxin.

So Nonselective Beta Blockers are good in cirrhotic patients but bad in SBP. Can someone explain the physiology of why NSBBs lead to worse outcomes in SBP? #MedTwitter #FOAMed
