@anaesthetic_spr I’m saddened by this whole debate. We desperately need more specialist training places for our resident doctors. We need more doctors but there is a place for non- medical practitioners/AHPs within MDTs. A physio ACP is valuable within e.g the MSK MDT. The skill mix is key.

ACPs bashing IMG doctors, thinking that paramedics, dietitians, OTs, physios, etc., are safer than IMGs. While ACPs are learning on the job with no formal exams, the average IMG doctor goes through: 👉🏻 Medical school with rigorous assessments for 5–6 years 👉🏻 IELTS/OET language exams 👉🏻 PLAB 1 👉🏻 PLAB 2 👉🏻 PSA exam 👉🏻 SJT 👉🏻 MRCP Part 1 👉🏻 MRCP Part 2 👉🏻 PACES

If student ACPs (paramedics, physios, podiatrists, OTs…) and PAs don’t study microbiology in depth, how are they allowed to prescribe antibiotics and antifungals, given the risk of hepatotoxicity? This prescribing course does not cover microbiology at medical student level. Discuss

Debbie: You are doing an amazing job working within your specialty/profession. I’m pretty sure doctors regularly refer patients to be reviewed by podiatrists who work closely with the vascular team. No one has an issue with what you do. We are concerned about some AHPs doing the MSc and cracking on with doctors’ jobs, claiming to be SHO/reg equivalent in ED, for example. This is unsafe. Ttots: Read above, advanced practice used to be within the same profession (advanced physio, etc.), but now it’s getting vile, with ED ACPs working at consultant level.

@anaesthetic_spr I follow local guidelines e.g. antimicrobial and hyperkalaemia. I’m as safe as any of us can be. I am not a doctor and don’t claim to be. I have real concerns regarding PAs and ‘fast track ACPs’. Please don’t tar us all with the same brush. Discuss?

@anaesthetic_spr I prescribe for a very limited specialist area based on C&S from deep tissue samples. I request and review bloods and regularly discuss with our MDT and microbiologists. I support our resident doctors with their prescribing decisions and teach on management of DFIs.

@DrPyaricetamoI In defence of my ‘proper’ diabetologist colleagues, they usually only get the opportunity to review their patients once or twice a year. Is it not reasonable to highlight obs of hypertension to GP (who will be the prescriber of anti-hypertensive meds) to monitor and treat?

Saw a 36 year old man today whose BP was 180/80 mmHg. He's been seeing a "diabetologist" since 7 years who's noted this BP since last 2-3 years or more but has neither evaluated or treated him further. When I auscultated him, turned out he has a grade 4 murmur with a proper thrill. His treating diabetologist has never put a stethoscope to his chest in all these years 😵‍💫

“The NHS needs every doctor it can get. But we now inhabit a world where doctors are unemployed. It’s madness. Please, @wesstreeting, do not be the health secretary in post while doctors are unemployed & signing on with Uber shifts. You do not want to be that person. Any health secretary in that position is doing something terribly wrong.” Did you know 20,000 young NHS doctors face not getting a training post this summer? That some are already unemployed - when the NHS needs every doctor it can possibly get. @DHSCgovuk needs to fix this immediately, now - it's a workforce debacle of monumental proportions & it's harming our patients. Thank you @lspencerelliot & @Independent for covering this hugely important issue today.
