@richardbogle Culprit could also be prox LAD giving collateral(s) to a CTO RCA (inferior Q-waves), now with no collateral filling, hence the inf ST segment elevation.

STE is in V1/V2 initially we thought ischemia in the LAD prox to 1st septal and D1. But that would show STE in aVR and depression in inferior leads, A closer look shows subtle STE in III and aVF compatible with acute inferior MI and the V1-V2 explained by RV involvement.
