Insurance companies love to claim they’re reducing prior authorization burden. Meanwhile, @UHC required me to bring a patient back after every Xiaflex cycle to “prove” he still had curvature — a completely non–evidence-based requirement. So I brought him back. Induced an artificial erection. Documented persistent curvature. Then the treatment was denied anyway. One reviewer claimed the curve wasn’t documented (it was). Another said the patient didn’t explicitly state the curve was “markedly improved.” Since when is Xiaflex expected to cure Peyronie’s disease after a single cycle when the FDA-approved protocol is 3–4 cycles? And of course, no peer-to-peer was offered. It’ll eventually get approved. All they accomplished was delaying care and pushing the drug cost into the next quarter. This is not utilization management. It’s obstruction.

Why the difference? ChatGPT learns from the internet, which includes misinformation and non-guideline recommendations. UroGPT was trained exclusively on expert-authored EAU guidelines, preventing unsupported recommendations and improving guideline concordance.
