Last year, Dr. Ezequiel Véliz was named 2025 Resident of the Year at UT Health, Rio Grande Valley. Last week, ICE detained him in McAllen, Texas—ripping him away from his family, his life, and his patients. The Trump Administration’s mass deportation campaign is destroying lives. ICE is not going after the worst of the worst, they are targeting people like Dr. Véliz, who has been living in the U.S. for nine years, married to a U.S. citizen, and meeting the healthcare needs of a severely underserved population. Dr. Véliz should be released immediately.

Just finished a surprise Congressional oversight visit at a temporary ICE holding site in Mesa, AZ. The conditions are absolutely horrific. No human being should be treated this way.

I don’t think people around the world realize how bad the attack on Lebanon was today. It’s BAD. Like 9/11 BAD. Hospitals are overflowing and do not have enough blood. We still don’t know what the death toll is. This is TERRORISM. Pure and Simple. April 8. Never forget.

Trump just requested a $1.5 TRILLION military budget. Last year, he cut $1 trillion from your health care—the largest cut in American history. But magically found $1.5 TRILLION for death and destruction. This is deranged.

Another peer-to-peer call. The reviewer was kind. Really kind. But, respectfully, not my peer. He listened as I explained why my patient needs lymphovenous bypass surgery. It’s a surgery that can treat the painful swelling in her arm caused by breast cancer treatment and prevent it from getting worse. She has done everything right. Compression. Therapy. Expensive garments. She’s changed her life to spend an hour a day with every resource she can afford. It’s affecting her ability to work as a nurse. And she is getting worse. This is the window where we can fix it. Early enough that surgery can actually change the course. We can do this surgery outpatient. Two hours. CMS even has a code for it. The man on the phone agreed it would help her… But he couldn’t approve it… Because he’s not a plastic surgeon and doesn’t have the specific knowledge necessary to make this recommendation. He gave me the help he felt he could. He told me to appeal and told me not to expedite it, because that wouldn’t work in my favor. Every human involved in this case knows what we should do. But the system is inhumane.

Universal health care access. $25 minimum wage for health care workers. $20 minimum wage for fast food workers. Free childcare — and an entirely new grade that saves families $20k+ / year. Free community college. $11 insulin manufactured by the state. Largest state tax rebate in American history. All while growing our economy to become the 4th largest in the WORLD. California defies the punditry of haters all the time. We’re focused on delivering results. And we’re not done yet.

I try to remind med students and practicing physicians who consider scaling back or giving up clinical careers for content creation how unstable it can be. People view content creation as freedom. You can work for yourself on your own schedule. That's true to a certain degree, but keep in mind that you don't own your audience. TikTok owns it. YouTube owns it. One change in the algorithm could significantly impact your livelihood. A few years ago, YT reclassified shorts as videos that are under 3 minutes instead of 60 seconds. Overnight, my social media income was cut in half. It didn't matter too much to me, because I still work full time as an ophthalmologist. I know I shit on the healthcare system and point out all the terrible things we deal with as physicians, but for all its faults, practicing medicine is an incredibly stable career, much more stable than content creation. I will never begrudge anybody for pursuing their passions. Some people just can't practice medicine anymore, and are looking for any way out they can find. Content creation is a way out, and hardworking, talented people will make it happen. I just want people to know the grass isn't always greener...

The day the Affordable Care Act passed was one of my proudest moments as president, because it meant that millions of Americans would have access to health care, some for the first time. The ACA also prevented insurance companies from denying people with pre-existing conditions coverage, allowed young people under the age of 26 to remain on their parents’ plan, expanded Medicaid, and so much more. But the ACA was always meant to be a first step. We still have to do more to expand access and make health care more affordable for everyone.
