Classification of Aortic Dissection Type I: Refers to a primary tear in the ascending aorta with dissection involving the aortic arch and descending thoracic aorta for a variable distance. Type II: Refers to dissection involving only the ascending aorta. Type III: Refers to a primary tear distal to the origin of the subclavian artery, extending distally for a variable distance.

@DrorLeviner @georgetolisjr Dr Schaff wrote up the issue with anticoagulation in 2001.

A bioprosthesis implanted on a young patient does not only expose them to the risk of a reoperation but more importantly to a second lifetime cycle of LV strain (aortic) or pulmonary hypertension (mitral) as well to development of endocarditis of the degenerated bioprosthesis.

@georgetolisjr So true. What is also true is that half of pts getting bio-mitral valve will still need anticoagulation for afib or other indications. 1/3 pts getting bio-aortic valve will.

@DrorLeviner @DrorMevorach ואפשר לבנות קאדר סביר של אנשים (כולל פמפיסטים, אחים, טיפול נמרץ) שיבנה מחלקה בצפת למשל? עם נפח עבודה סביר שישמר מיומנויות שרכשתם בהתמחות ובהשתלמויות בחו״ל? אבל אם רוצים מרכז טראומה זה בעיקר שיהיו כירורגים חזה ונוירוכירורגים. לא חושב שיש מקום למחלקה שלישית (ואפילו רביעית) בצפון.

מה משמעות ההכרזה השבוע על בתי חולים צפת ואשקלון כמרכזים על אזוריים לטראומה? הדבר יחייב פתיחת מחלקות נוירוכירורגיה ולב-חזה. מהיכן יגיעו הרופאים? או שיגיעו צעירים מוכשרים שיקבלו הזדמנות או שיגיעו לא צעירים שלא מצאו את מקומם עד כה, או פנסיונרים. כך או כך למחלקות תהיה עקומת לימוד והכשרה שתמשך שנים. ומה תעשינה המחלקות מעבר לטראומה? לב-חזה לצערנו מחלקה בירידה דרמתית עם פחות ופחות אנדיקציות לניתוחים לאור השתלטות קרדיולוגית כך שהיום יום במחלקה לא נראה מבטיח במחלקות אלו. נוירוכירורגיה מחלקה שיש לה מה לעשות אבל לא בטוח שמבחינה ארצית צריך תוספת של שתי מחלקות כאלו. כל זה נופל על הקושי לאייש רופאים בפריפריה. בדרום יש את סורוקה הקרובה ובצפון את רמבם הקרוב. ואגב מה עם נהריה?

@DrorMevorach יותר מבוגר (ואפילו לימדת אותי בפנימית לפני אי אלו שנים). אני מדבר על עשר השנים האחרונות פחות או יותר. צריך פחות ממה שיש בארץ, זה בטוח. כמו בהרבה תחומים יש קשר ישיר בין כמות הפעולות לתוצאות ולכן עדיף לרכז את הפעילות בפחות מחלקות

@DrorLeviner תודה. אני מניח שאני קצת יותר מבוגר ממך ועוד זוכר את פעילות המעקפים והמסתמים שהיתה פעם… ולכן דרמטית. מסכים לגמרי שזו מחלקה נדרשת מאד וחשובה מאד אבל השאלה כמה מחלקות כאלו צריך.

@DrorMevorach בלתי ישים. לא יקרה. יש מעט מאוד כירורגים חזה בארץ, אין מספיק כירורגית לב. גם עם נוירוכירורגיה יש בעיות ובלתי אפשרי יהיה לבנות עוד שתי מחלקות עם מספיק רופאים לכל קשת הטיפולים הדרושים ממחלקה כזו. לא יקרה. ואם יקרה - יפגע ברמת הרופאים שלא ינתחו מספיק

יש לי בקשה קצת מוזרה אבל אשמח ממש אם תשתפו פעולה וגם תפיצו הלאה - כל יום בשעה 15:00 בדיוק סשה טרופנוב היה מכריח את כל הצוות שלו באמזון לעזוב את המקלדות, לקום מהעמדה ולבוא לשתות איתו קפה. זה היה מבחינתו קודש הקודשים וכולם היו מצייתים - דניאל, אילן המנהל שלו, כולם. אבל כבר יותר מחודש שהבחור המקסים הזה, ומהנדס על של ממש, לא שותה איתם קפה בשלוש. הוא נמצא בשבי החמאס עם אמו, סבתו וחברתו - למעשה, כל משפחתו בארץ. יצא לי לשוחח בשבוע האחרון המון עם חברים של סשה לצוות, והם אופטימיים ומאמינים שהוא ישוב, אבל לתחושותיהם בגלל שכל משפחתו איתו בשבי אין מי שיחשוב עליו פה בארץ, וכשהיום המיוחל יקרה והוא יחזור לישראל ולעבודה, חשוב להם להראות לו שאנשים כן חשבו עליו, שלאנשים היה אכפת. אז הנה הבקשה שלי - בבקשה תכינו היום קפה ב-15:00, תחשבו לרגע קצר על סשה ומשפחתו, ותעלו בציוץ נפרד או בתגובה לציוץ זה תמונה של הקפה (או שלכם) עם ההאשטאג Coffe4Sasha#. אני וחבריו של סשה לצוות נאסוף דרך ההאשטאג את כל הפרסומים, ונראה לו את כל התמונות אחת אחת ברגע שהוא ישוב לביתו. מבטיח. תודה.

60M. Known asc aortic aneurysm. Now onset of new chest pain so sent to CTA (below). New diagnosis of PAU ➡️ sent to surgery. Underwent asc aorta replacement with a 30mm graft

Interesting how many 3.8cm ascending aorta referrals we see but somehow the really big ones slip through the cracks. Did a 3rd time sternotomy, hemi arch, root replacement on this 7.2cm ascending aorta patient (prior BAV). What other large ones have you guys seen?
