Sample transaction form below. 3-If you prefer to submit forms with your broker account numbers redacted [you must include the last four digits], you need to submit your POI form unredacted [sample below], a redacted version, a NOTICE OF REDACTION [sample below], transaction data [dates, shares, monies invested], proof of holdings [broker/transfer agent statements], and $28 fee. Include your certified bank check, money order or personal check pay to the order of "US Bankruptcy Court". ▶️INFORMATION SUBMITTED ON/WITH YOUR POI IS SUBMITTED UNDER OATH, UNDER PENALTY OF PERJURY.
