This week’s ED documentation tip: Document the decision process for diagnostic tests considered but not performed. Kid w/ a head injury, PECARN says no CT? Ottawa rules say no x-ray for that ankle? Data scores the same as ordering the test with your thought process documented.

This week’s tip: Include “per my interpretation” when documenting your interpretation of EKGs, X-rays, ultrasounds, CT scans, and rhythm strips. Getting Category 2 credit for independent interpretations will be crucial for reporting 99285 for discharged patients in 2023.
