Please stop telling people with osteoporosis not to lift anything heavy... I've heard it from docs, PTs, Trainers, etc... This might seem protective... but it's not. This risk calculation... A hip fracture in an older adult carries a one-year mortality rate of roughly 25 percent. Half of those who survive never walk independently again. That is the cost of fragility. The risk of a well-supervised, progressively loaded heavy (80-85% 1RM) strength program (LIFTMOR) is a muscle strain or a bruise. These are not equivalent risks, and treating them as equivalent, as avoidance does, is not caution. It is a decision to accept the larger risk in order to avoid the smaller one.

Consent ✅ Runner - Relative Energy Deficiency in Sport (REDS) and prior eating disorder High grade calcaneal stress fracture (with almost completed fracture line) - see MRI image Immobilised in Aircast boot (ACB) for prolonged period based on persistent heel pain, despite follow up MRI showing appropriate healing and regression of bone marrow oedema Patient adamant nature and location of pain now different Now - burning, ‘horseshoe shape’ around heel and radiating into lateral plantar foot (see patient's pain map; red - medial, blue - lateral) Finally, out of AirCast boot and off crutches after third MRI - but pain persisting on return to longer walks and jogging Found amitriptyline helpful in modifying what is clearly neuropathic pain in the interim OE - Calcaneal squeeze negative Tinel’s along lateral plantar nerve (LPN) and anteromedial heel (Baxter’s nerve (BN) inferior calcaneal nerve course) very provocative Images from first MRI (axial T2s) show LPN oedema and possibly BN which are immediately adjacent to the areas of calcaneal periostitis and inflammation POCUS video of hydrodissection with low potency soluble dexamethasone and local anaesthetic (note - LPN thickened at point of maximal tenderness, provoked with hockey stick probe pressure) As injecting – “Ouch! that’s the exact pain I’ve been having” At follow up pain intensity reduced by 60-70% despite tentatively weaning off amitriptyline For second hydrodissection, this time with 5DW

Consent✅ Gaelic football Longstanding posterior pain - epicentre is principally inferomedial Achilles Several pain 'culprits' on clinical assessment, MRI and ultrasound - Plantaris compressive tendinopathy (inserts very low medially in this athlete) Insertional intrasubstance Achilles tendon tear Posterior ankle impingement - signs on MRI including os trigonum and posterior recess fluid Process is critical if covering all bases in one session (see POCUS video reel)- 1. US guided diagnostic LA injection, out-of-plane approach, to posterior ankle recess (before regional anaesthesia block which would 'cloud' the diagnostic assessment) - can just make out needle tip at the deep joint line No change in pain on calf raise and hop - therefore its not the ankle joint 2. Then tibial and sural nerve block to give good regional anaesthesia around mid and distal Achilles but to avoid motor blockade (would happen if did popliteal block more proximally) 3. Once patient completely comfortable and insensate, high volume stripping of the plantaris tendon (LS) 4. Post injection TS video shows plantaris separated from main Achilles tendon body 5. Further mini scraping procedure of the plantaris- Achilles interface with wider bore needle bevel 6. Process finished with PRP injection to intrasubstance insertional Achilles tear - can see injectate passing quite a distance proximally (these occult defects are common, including in other tendons eg lateral elbow, patella tendon & plantar fascia) Post procedure - Keep area clean & dressed (multiple needle passes) Aircast boot, small heel raise Gradual wean and re-loading of Achilles under supervision of experienced lower limb physiotherapist A lot done in 45 minutes!
