Intramedullary spinal cord tumor. This young man from across the border recently came to me with acutely worsening weakness and severe spasticity. I did a revision surgery gross total resection. Previously operated in 2021 at some other centre, they did a biopsy and sent for XRT.

The spinal cord is a scary thing to incise. The cord has a midline fissure that makes it easier there, but paramedian cord does not. However, myelotomy along dorsal root entry zone (DREZ) is safe for dorsal spinal cord cavernous malformations, entering posteromedial to descending corticospinal tract & dancing around posterior spinal artery…

Time flies! A year of amazing growth & many 1sts for the institution (1st mobile spine en bloc spondylectomy cases, 1st total sacrectomy, 1st MIS intradural tumor resection, etc). I thank God for the wisdom and technical ability to care for complex patients!! Much more to come!!

Patient with MRSA spondylodiscitis and osteomyelitis from C1-6 with failed medical therapy over 6 weeks, causing severe cervical stenosis. I had no other options other than to fix from C0-T2 and decompress from C2-5. Patient sent home walking on 6th post-op day.

TLIF done in a 45 year old with spondylo-listhesis and lumbar stenosis. Patient doing well after the surgery.

The prevalence of intraspinal epidermoid is 0.5-1% in the literature! Pre and post op scans I ll share tomorrow!

Epidermoid vs dermoid intradural tumor? Another interesting intradural spine tumor of a 30 year old lady, I did yesterday. Two intradural tumor cases within a span of 5 days. I was able to get complete resection The patient has no neurological deficits post-operatively.

Intradural tumor I operated yesterday: A 22 year old gentleman presented to me with right lower limb pain and numbness for 6 weeks. MRI showed a 2.8x 1.3x 1.7 cm right sided intradural tumor at L4-5 level. I did a laminectomy and resection of this tumor.
