🤩 What a great 🔥 chapter, this #EBJIS2024 congress in Barcelona 🇪🇸! What a pleasure for me to belong to this magic board ✨ that includes orthopaedic surgeons and infectious diseases physicians friends from so beautiful countries that make Europe what it is: diverse, respectful, friendly, productive, and so human- and team-based! It was an honor for me to report how #phage therapy could help for the treatment of patients with prosthetic joint infection (#PJI) in the era of multi-drug resistance! What a crucial question, as antimicrobial resistance is a tsunami 🌊 leading to limited treatment options, and severe individual consequences. #Bacteriophages are clearly an option, as adjuvant, to face this huge problem. I reported the literature review that we recently published this month with the team of my colleague and friend Pr. Sebastien Lustig (see here ➡️ link.springer.com/article/10.100…) and presented how we managed the 4 patients who were treated at our institution Hospices Civils de Lyon - HCL 🇫🇷 with phages from @PHAXIAM, from @BiomX_Inc and from academic sources. Indeed, for a patient infected with a multidrug resistant Staphylococcus epidermidis and for whom no phages from the industry were available, a phage isolated by colleagues (🙏) at @inselgruppe in Bern🇨🇭that looked to be active was then purified by the Queen Astrid military hospital in Brussels 🇧🇪 (🙏) before being mixed in an active cocktail that was successfully administered in 🇫🇷! I presented also the various therapeutic schemes we developed and currently applying depending on the patient clinical presentation. The standard exposure (Std-Ex.) is now three injections with a one week interval, a scheme that has been included, based on our experience, in the next clinical trial promoted by @PHAXIAM. I presented also the clinical case of patients who received or maximum exposure (Max-Ex.) that includes intravenous infusons of phages and a total of 9 local injections during a therapeutic scheme of three weeks! All of that was possible thanks to the indefectible dedication of my colleagues (🙏) and to the support of the @FondationHCL to our program #PHAGEinLYON_Clinic that aims to make #phagetherapy possible and more and more common in Lyon! We will go ahead now to publish soon our experience, and it’s absolutely sure that there will be more and more talks and works on the topic of phage therapy at #EBJIS2025 @EBJIS_ in Bologna 🇮🇹! Our patients crucially need this development, and treating them is definitely like a piece of art 🎨 or a jazz 🎶 partition played collectively: it requires inspiration, motivation, training, interaction, improvisation and succeed in showing that it works! 🙏 Thanks to all of my friends from the board, and to the @CapPartner team for the incredible interactions we have! 😀 I'm delighted to have been able to treat with #phages this burned ICU patient, cared for by my intensivist colleagues at @CHUdeLyon Hôpital Edouard Herriot, and who was suffering from recurrent extensively drug-resistant #Pseudomonas aeruginosa ventilator-associated pneumonia (#VAP) that was threatening his vital prognosis mdpi.com/1999-4915/16/7… Phage therapy is promising in severe bacterial infections due to multidrug- and extensively-drug resistant P. aeruginosa, but very few patients can be treated in the #ICU at this time. In fact, there is a long time lag between isolating and characterizing bacteria, thinking about #phagetherapy, sending the strain for in vitro testing of phage activity, selecting active pharmaceutical-grade #bacteriophages, and receiving them for clinical use. It could take a minimum of 7-10 days, that is frequently not compatible with treatment in emergency. Here, phage therapy was made possible thanks to the experience acquired at @CHUdeLyon in the treatment of joint prosthesis infections at @CrioacLyon, by the development of the #PHAGEinLYON_Clinic program supported by the @FondationHCL, by the proximity that we have as infectiologists with intensivists, and by the fact that we were able to anticipate a potential relapse of this serious infection. Indeed, the patient was able to be treated rapidly at the time of the relapse, as we had previously assessed the possibilities of subsequent phage therapy during the first episode. Thanks 🙏 to all people who participated to this effort! Thanks to the #PHAGEinLYON_Clinic program, at this time, we have so far treated 84 patients at HCL, including 4 serious pulmonary infections. There is indeed a need to extend phage therapy beyond bone and joint infections, notably as an adjuvant treatment for severe bacterial infections such as infections in intensive care units, infections on vascular implants, endocarditis and in some chronic lung infections. This experience is currently crucial, is a considerable hope, is being valorized, and will be reported at the @ESCMID @esgnta workshop to be held in Lyon on November 12-13, 2024 (program: escmid.org/fileadmin/escm…; registration: escmid.org/event-detail/s…). "Black 😱 Thursday" or "Incredible 🤩 Thursday" 📆? Each Thursday, since many years, I am managing the 1-day multidisciplinary hospital dedicated to complex bone and joint infection (#BJI) at @CHUdeLyon. It is one of the main activity of our referral center called #CRIOAcLyon (crioac-lyon.fr). It could be the "Black 😱 Thursday", as we are receiving each time 15-20 patients, who are coming sometimes from far, with very difficult situation that we have to manage with humility. It is moreover not easy as we have few time to welcome them decently, whereas they are ill and most of time severely disabled. We have to be available to hear their story, whereas they are suffering, to perform examination, imaging, blood test, dressing, and then to take the best medico-surgical decision for them! In fact, it is really the "Incredible 🤩 Thursday"! First of all we found an optimal organization with first examination during the morning by infectious diseases physicians, and then performance of a multidisciplinary staff at 2 p.m. that includes discussion with the personal input of each specialties! What a pleasure to see >10 professionals with their computer, fully concentrated to hear each individual history, analyzing what they could add, and concretely participating to the decision we are taking together! Learning from each other is so awesome, as the direct impact for the patient is really perceptible (for instance, a pleasure for me to share my vision about #phage therapy, and at the counterpart hearing and understanding how to perform a total femur implantation is impressive)! We were also so happy to welcome this week Salomé and Olivia, two 15-years-old teenagers 👭, for their work experience internship, as they would like to discover our jobs! They brought us a lot of happiness and motivation, and it was fantastic to see their enthusiasm and to see their eyes full of stars 🤩! They participated to the management of the patients, they saw injection of #phage under sonography, they participated to discussions, and also to the multidisciplinary clinical round! I'm very keen to share with the new generation the importance of keeping the patient at the centre of our efforts, but also the importance of ethics and respect and professional conduct among colleagues, and the crucial role of what everyone contributes at their own level, both at the paramedical and at the medical level! All of that facilitates the set-up of innovations, but we frequently need to accept some compromise in the intervention we would like to propose and sometimes unfortunately we have to accept that we failed to cure the patient because of superbugs. In the end, and more importantly, I hope we always succeed from a human perspective with such a spirit and with the sincere involvement of everyone here!  I acknowledge sincerely all my colleagues, I love working each Thursday with all of you 💙 (especially in this difficult time for me)!

💫 It was a great honor to participate in Tbilissi this week to the 2024 France Alumni day organized by the French Ambassy in Georgia 🇬🇪! We were welcomed by Madam the French Ambassador in Georgia @Sherzag and his team who organized this nice event dedicated to health! We reported the cooperation that we have at @CHUdeLyon, @UnivLyon1 and @LeVinatier in Armenia 🇦🇲 and that we have developed in trauma patients and in the field of bone and joint infections since the 2020 war in Karabagh. We were connected in Georgia 🇬🇪 with our infectious diseases colleagues from the "Groupe Hospitalier Sud Île de France" located in Melun, and with colleagues from 🇦🇲 and Georgia 🇬🇪, and we discussed collaborative projects we could have in the patient care. Of note, it also includes also #phage therapy, especially as there is a huge heritage about this therapy in Georgia 🇬🇪 at the Eliava Institute that we have to not loose and that we have to develop to face complex clinical situations and antimicrobial resistance. Understanding when and how to use it in infected trauma patients remain a crucial step from my point of view! It is clear that in the current context, people and colleagues in Armenia 🇦🇲 and Georgia 🇬🇪 deserve our full attention and deserve that we take the time to understand their practice, with the aim to improve their capacity of develop an as optimal as possible patient care. The French spirit in healthcare we wish to defend is composed by universal values that it are essential to pass on from our point of view: humanism, humility, fair access to care, sharing of knowledge, multidisciplinary care and involvement of professionals who need to get to know, respect and appreciate each other, and work together towards a single goal: putting the patient at the centre of all decisions to improve the quality of care! By being connected with France Alumni, Campus France, the "Institut Français en Géorgie 🇬🇪" and the "Institut Français en Arménie 🇦🇲", we will generate for sure ambition in our West Caucasus students in health and Armenian and Georgian colleagues to learn French and to choose France 🇫🇷! In the current context, sure it is the best choice in the perspective of the "common good"!
