@GleefulKaz @neilmo @BenXGowland Patients need a voice at every level. Practice, PCN, Federation and system to co-design pathways and ensure what really matters to individuals is at the heart. Shared decision making.

The number of PCNs has risen by 57 over the last 3 years (1250 to 1307). I would expect the number to grow a bit more sharply over the next year or so, as practices seek to align themselves more closely with their PCN.
