Burr entrapment, one of the most feared complications in rotational atherectomy. Multiple bailouts, choice driven by operator experience. Algorithms help organize the toolbox, especially for less experienced operators. My usual path: step (2) vasodilator + gentle pull (solves most), then (5) wire + balloon next to the burr or (6) knuckle wire for the resistant ones. With step (3), skipping rope, a newer iterative technique, I'll probably start following this more logical sequence. #CHIP #Rotablator #RA #atherectomy #entrapment

This week marks an important milestone in the evolution of CT-guided PCI with the release of PCI Navigator for clinical use—a purpose-built application for CT-guided PCI planning that enables interventional cardiologists to directly interact with CT data, including disease morphology, FFR-CT–based physiology, myocardial mass, lesion length, vessel sizing, and projection planning. These concepts have been evaluated in the P4 randomized trial (@ColletCarlos), with 1-year outcomes anticipated later this year. PCI Navigator will be evaluated in our investigator-initiated CT-PLAN PCI randomized trial (@MHIF_Heart @esbrilakis) and in the NAVIGATE-PCI registry (@ziadalinyc @ToddVillinesMD).
