Period cramps (dysmenorrhea) are exclusively painful uterine muscle contractions during menstruation & has nothing whatsoever to do with estrogen. Trans women can absolutely have bad reactions from drug estrogen, however.

There are perisex cis women who experience period cramps even after their uterus and ovaries are fully removed. Period cramps aren’t just caused by the uterus and ovaries, it’s the hormones themselves too 🤦🏽‍♀️ So yes, trans folks on estrogen can experience period cramps.
