@bryceolson @SarahE_Richards Olaparib and BRCA2 synergize. Ola inhibits NHEJ repair and BRCA2 reduces the effectiveness of HR repair (DNA DSBs are almost exclusively repaired through NHEJ or HR). Interesting results for BAT/olaparib. Seems to work regardless of HR mutations. But it was a small trial.

@JJDomDad @SarahE_Richards Thanks! I’ve heard same, especially for BRCA2 mutant patients. I did BAT already and had quite good success for several months, but nothing has proven super durable at this point.

Update: @bryceolson is on his 15th line of therapy since his 2014 diagnosis. He’s recently had success with bipolar androgen therapy and is now bouncing between testosterone and anti-testosterone therapies until a better option comes along. Perhaps a mRNA vaccine for cancer?

@BatPitt That's too bad that your MO has never heard of BAT. I wonder if they spend much time researching. It is one of the hottest topics in the PCa front. I think pcf said it was #1.

@JJDomDad I went to MO today. I refused Enza in July at PSA 8. I went to an RO and After SBRT to oligomet, now PSA=2 MO asked if I did something else than SBRT. I forgot to say 1 weekly Dukoral. I asked for BAT to lower PSA <1. MO never heard of BAT ☹️ said won't experiment with testoste

@BatPitt I also had the Dukoral. I don't know if the observational evidence is correlation or causation. But I was in Seattle so a trip across the border was easy. And I figured not much to lose and lots to gain.

@JJDomDad There are trials ongoing with Lu177 + pembro; BAT with parp inhibitors. Mixing all 3 would/will be better imho. Trials involving TiLs finally coming too

@BatPitt Does your MO support it? It is experimental. I took the plunge and so far it seems to have worked well. 15 months and PSA is dropping and PSMA-PET scan is clear.

@JJDomDad I have PSA progressing on ADT Zoladex that I started in March 2019. I just had my 3 months belly dose yesterday. Now I read this in one of the links in your book. I am considering doing BAT before start taking Enza.

@laurasenamd Thanks. The BAT/Xtandi trial is one that I am watching closely. I'm using propionate. My MO and I are doing to start Xtandi/BAT/X/B... if I fail BAT.

2. In patients and PCa models with high pre-tx ARA, downregulation of c-MYC contributes to growth inhibition by BAT. We saw huge decreases in MYC in some pts! This is notable because MYC is a common driver of advanced PCa and is considered by some to be "undruggable"

@laurasenamd I use propionate instead of cypionate so that I can get down to a true castrate level.

@laurasenamd Nice graphs! I'm Gleason grade 5, T3c, N1M0 and have been doing BAT for 14 months. My bone density per DEXA has increased 5.20% absolute. My TMB has dropped. My PSA has gone from 0.17 to 0.02. I had a Pylarify PSMA-PET scan a month ago and no signs of cancer were found.
