A heart breaking truth about end of life care in the US. Home hospice does not provide daily support and is for minimally symptomatic. Inpatient hospice is for the actively dying. The gap between these scenarios is vast and people suffer.

As a physician, I wanted to find a PCP in my own health system. There were no appointments near my hospital. None. So I went for a clinic halfway between my home and the hospital. Not ideal, but it will do. All the other clinics are on the far north metro, far from both. 1/6

Bad news is the gap between the patient expectations and reality. Any information could become bad news. As long as you talk to patient and family, it is IMPOSSIBLE to avoid giving bad news. If you say you don’t have to, you are either not talking to patient or lying.

Making a recommendation is not the same as being paternalistic. We are helping to interpret their goals/values, and explain how to achieve that. This is a privilege we have as a clinician because we studied/trained so hard.

The hardest part of my job is preparing young patients, who are simply not and often never ready, for death. When to say no when they are fighting for more treatment. Preparing their families who are in denial. All in a collapsing mental health/social work infrastructure. 😔

A key to regulating emotions is to treat them like visitors in your home. "I'm angry" lets a feeling take permanent residence. "I'm hosting some anger today" is a reminder that not all guests are welcome. Emotions may drop in unannounced, but we decide which ones get to stay.
