@tomgoom Is there any info on the posterior knee tendon injuries? Or is this chart just based upon anatomy and the plausibility of tendon pain?

Posterior knee pain differential diagnosis. I’ve focused on the more common injuries but it’s worth noting that bone stress injuries (such as tibial stress fractures) and bone tumours can also cause posterior knee pain, although they are more rare.

Hot take, the Quad Tendon ACL autograft needs to go away... I have not seen ANY evidence indicating benefit greater than HS or BPTB autografts... On the contrary, QT graft knee extension strength tends to lag behind its autograft counterparts. THIS IS NOT GOOD!!!

Random shower thought this morning! I know many docs are shifting to QT and one reason cited has been knee OA in patients getting a PTG graft. If that’s true, are contralateral graft site knees becoming more arthritic than a normal knee? Curious what Dr Shelbourne’s group has seen. He’s done contralateral PTGs for many many years and would be seeing that, no?

Second patient of the day: "Let's drop down to once next week, seems kinda crazy to pay someone to make you exercise." Me: 🙂 Literally 1 week ago, he couldn't walk for more than 10 minutes. He just walked for 20 minutes yesterday. Seems kinda crazy... k, done venting

The Reading and/or Levy shoulder programs are just money for massive RCTs. Got a guy who couldn't elevate to 100 degs w significant UT compensation and can now get to 165 w both abd and flex. Definitely my go-tos for this population. Hopefully will avoid or prolong surgery.

Don't think I've ever seen this before, but I'm fairly certain I have a guy w/insertional glute max tendinopathy. Competitive weightlifter & powerlifter. Symptom pattern & pain location makes sense given high squat demands. Seen this @DrPeteMalliaras @kgSilbernagel @ProfJillCook?

@kcrehabguy Self pay, adolescent volleyball player. Described knee valgus MOI. Drawer, Lachman and lever sign negative. She has a little patellofemoral pain to address. Trying to prioritize rehab for prevention. I’ll be lucky to get 2 follow up visits.

@dracheypt @kcrehabguy Patient had been seen by multiple physicians locally before arriving in my office. And then she eventually spent a week at the Mayo Clinic... But, if you have enough lumbar MRIs they'll eventually find something And, she doesn't even have pain lifting overhead! 🤷‍♂️

@kcrehabguy Thanks for all of the twitter support:) There is a lot more to the story--the patient has seen at least a dozen Physicians, insidious pain at multiple locations. And now she has a phobia of overhead lifting...🤦‍♂️

@kcrehabguy The Aflredson protocol is shit ton of volume: 3 sets of 15 heel drops, 2x daily. You're supposed to add a backpack full of books to progressively overload it. How bout an undulating program? Heavy one day, and higher rep or isometric the next day. - That's what I do.

Reviewing these babies in journal club next week. Should be interesting! ACLD diff than ACLR but Chius previous paper advocated for posting uninv. Always curious if Alfredson protocol done w higher loads. Too light to me. 🤔
