With this in mind, in a @LeedsHospitals oncology project we have re-named our frailty assessment as a 'wellbeing assessment' - much better received by patients - but we're still figuring out how to best discuss frailty with patients once identified @dswinson3 @sherena_nair

@sandradyer_05 100%!! Assessment of frailty should start as soon as a patient presents with cancer symptoms (often in primary care!) + community teams have a key role in holistic care for patients affected by both cancer and frailty
