I still cannot understand why people replace regurgitant bicuspid aortic valves in young patients. This patient had seen multiple specialists who recommended aortic valve replacement because of calcium on the fused cusp. Please repair or refer!! @AortaEd @mghachd @MGHHeartHealth

Not every bicuspid aortic valve is created equal — and not every one can be saved. This specimen tells that story-->Severe asymmetry, thickening and morphology with poor long term durability. The valve had to be replaced and these decisions can only be made in the operating room. What would you do? @GebrineK @JordanBloomMD @BavariaMd

No one with less than stellar results is willing to publish their dirty laundry. And often the definition of a high-quality surgeon is retrospective (which represents circular reasoning)

Tom thanks for your comment. There is a wealth of data on the topic and many series showing excellent long term outcomes and more importantly normal survival after valve repair. As with most nuanced procedures patient selection and surgeon volume are critical to good long term outcomes.

@JordanBloomMD @AortaEd @GebrineK Has anyone ever proven the rationale behind aortic valve repair? I’ll admit selection bias, but I’ve ever seen one last even a few years (or even less) without developing problems (AR).

Thanks so much for your response. I know that the Gore-Tex suture can cause early valve calcification when used for free margin suspension, which obviously puts lots of suture in the cusp and in particular at the rotation point. A simple interrupted Gore-Tex used for plication. I’m not certain would have the same effect. I look forward to hearing what others have to say about this. I don’t have the suture type that you’re referring to which appears to be a nylon, but I would have a similar concern. I guess one option would be to use a smaller bore suture, I have historically used 5-0.

@JordanBloomMD @AortaEd @GebrineK I used to use CV7 Goretex suture but stopped as I have heard that it causes leaflets to calcify. I was using prolene after that but now use Cardionyl 5/0 or6/0 and it works well and so far there has been no issues.
