Taking my second 19mm mechanical valve of the week out today for severe patient prosthesis mismatch and panus. Patients always report being told that mechanical valves last forever...

This is a terrific review of the Ross Procedure. Any medical student/aspiring CT surgeon attending the TSMA webinar "Aortic Valve Options: Why Ross?" on Jan 8th with Dr. Calhoon should read this paper before the meeting. @ThoracicStudent

Our newest @JAHA_AHA review on the #RossProcedure is out today... a surgeon/cardiologist collaboration offering a modern look at patient selection, surgical technique, and long-term outcomes to guide practice. @IHamamsy @OuzounianMD @LucyLNam @ddefariayeh @MGHHeartHealth @mghachd @AortaEd ahajournals.org/doi/10.1161/JA…

@StephaneLeung Got it...for me the quality metric here is not just "No AR". If: (1) effective height is >7-8mm, (2) zero residual prolapse, (3) 1+ or less central AR and (4) single digit mean gradient... can be safely accomplished, i would 100% choose VSRR. EOA probably 3x a tissue valve

@JordanBloomMD I don’t think there’s any problem, other than that some would just suggest that VSSR with BAV will fail with AR and then there’s no TAVR option (right now). So better off with bentall for future ViV.

Fellow VSRR surgeons...how would you deal with this 6cm root aneurysm in the setting of an asymmetric trileaflet valve with moderate regurgitation? Any tips for managing the asymmetry? @AortaEd @GebrineK @schaefers_hans

Great new paper in @annalsthorsurg by François et al. redemonstrating a critical principle in VSRR. You must get it right from the beginning. Unacceptable to leave the OR with more than mild AR and any degree of prolapse. Do it right or you will be sorry.

A number of years ago when I was in training, I was told by a number of surgeons that if I decided to go to a community practice, I will only be seeing straight forward CABGs and an occasional valve replacement. Below is our typical “community practice” week in South Alabama.
