TecLille: Tec+Dara in transplant ineligible NDMM n=37; median age 73 (!) Very high MRD negativity rates even at 6 months! Very interesting correlatives, including clonotypic peptide sequencing and sBCMA. No progressions. No deaths. 14% grade 3+ infections (nearly everyone got IVIg starting within first cycle). If this regimen can be so well tolerated upfront even in older adults, that bodes very well for the future of bispecific antibodies in frontline therapy.

As a 1st-year med-onc fellow in 2020, I received the Ladies Leukemia League (LLL) grant. Despite its modest sum (~$30K), it was pivotal for my research and securing further funding. Fast forward to 2024, LLL once again believed in a new idea of mine, uniquely funding it.

Myeloma drugs — Dosing and schedule What I do in practice #MedTwitter 1. Dex: 40 mg once a week age <70; 20 mg if age >70 or frail Stop Dex at around one year if good response. Days 1-4, 8, 15, 22 schedule for 1st cycle only in cast nephropathy; or VDT-PACE type regimen
