Those who believe in “psychological pain”, “brain pain”, “conditioned pain”, “pain without nociception” etc., are not realizing that their main premises are based on the appeal to ignorance fallacy. Or in other words - “abscence of evidence is not evidence of abscence”. Just because we cannot identify sources of nociception doesn’t mean they are not there and that the pain is “psychological”. Another implicit assumption they make is to assume that our imaging modalities are finite and are capable of capturing all possible nociceptive states which is far from true.

It's about systemic problems: exploitative labor conditions, poor supervision, job insecurity, and institutional neglect. If you're struggling with your PhD, it's not your fault. These are predictable responses to broken systems.

@DrSethPT @AdamMeakins @JOSPT These level of evidence and grade of recommendations does not match what is used in the rest of the world - which is why they can end up with - you must/should do dry-needling. In Denmark this could elicit a malpractice claim - if this the only intervention applied.

@AdamMeakins @JOSPT The grade relates to the underlying evidence, and is established beforehand, not the effect or how important it is to clinical care. I think people should understand that. No one would say do dry needling not education. It’s just about the levels of evidence.

If you think massage and dry needling are superior treatments for hip pain than education and weight loss just because @JOSPT said so… you’re deluded! 🤣

Officially published💥 A reboot to the pain field! The aphorism that “nociception is neither necessary nor sufficient for pain” has caused a lot of damage to patients and set back the field. This paper was not easy to publish and was rejected by all the major pain journals for obvious reasons of bias and suspected vested interests. Eventually, BRAIN which has more than double the impact of any pain journal identified the true potential and implications of the paper 💪🏻 The paper is a systematic and critical review. The systematic part identified that all those who cited the aphorism had no references to back this claim and they mostly just cited themselves circularly. The critical part covers EVERY piece of supposed empirical evidence that was ever presented as evidence for pain without nociception and shows that none stand critical scrutiny. The main and most important implication of the paper is that if we accept that “Nociception is necessary but not sufficient for pain”, then all pain states are fundamentally explainable and have somatic basis. The notion of “psychological pain” or “brain pain” should be shunned and binned.
