Why is every restaurant I go to absolutely a 5/10? Average food, average-to-poor service. I can't tell you the last time I went to a restaurant and thought, "This was amazing, I'll definitely be back." Anyone else?

I was young in the 80s/90s in the UK. I don’t remember my parents ever going out to eat, except when we were on holiday (in the UK). I don’t think they ever took me to a fast food restaurant, or ordered takeaway food. People today have no clue how working people lived. /1

Boomers seem to forget that fast food used to be a normal, everyday expense for them because it was affordable. You could get a burger easily on minimum wage, it wasn’t some fancy treat you had once a year as a reward for pulling yourself up by your bootstraps.

Senior Coroner in Nottingham Mairian Casey actually had meetings with Senior Management Team at Notts Trust to share her grave concerns; highlighted was ‘lack of risk assessments’ yet the SMT/Board just kicked the can down the road. The whole SMT have given evidence that has been shocking, agreeing they didn’t do enough and under their stewardship CQC ratings actually fell. They should all be sacked and they shouldn’t be allowed to shuffle around the country, get jobs on other boards and just continue enjoying life. Why is there no regulation, no penal element for their failures. They are the reason mental health related homicides happened in Nottingham and they are the reason it all exacerbated in the Nottingham attacks. When and how will they be held accountable? What is going to be their punishment? I’m here with a vacuous life without my murdered daughter who was fighting to protect her friend and there is no punishment for people who just didn’t do their job……..

My wife and I have worked in the NHS and have done so for over 25 years. Grace was a medical student and attended lectures at Queens Medical Centre, one day she would have qualified and started her chosen profession in Nottingham, treating the people of Nottingham, working in the NHS hospitals there. Psychiatrists are increasingly concerned about the risk they face in terms of accountability, particularly in the context of patient safety incidents. They are worried about being held responsible for catastrophic failings that occur under their watch, where patients can take their own lives, many of which are related to coercive prescribing habits. Psychiatrists should be worried. They aren't restoring or repairing lives. They're damaging and destroying lives. Psychiatrists, their Trust employers, and their membership body, the Royal College of Psychiatrists, consistently ignore the paradoxical effects of prescribed drugs, effects such as suicidal thoughts and suicidal behaviour. Instead, they blame a patient's mental difficulties for a catastrophic failure, while also lamenting a lack of public funding to carry on their "work". This modus operandi creates a smokescreen designed to deflect attention away from psychiatric failings. Psychiatry should be no different from other professions which take public and/or patient safety in the highest regard. This message was sent out during and after our fight for the Nottingham Inquiry came to fruition and when, being a doctor, I asked for ‘accountability’. I knew about this message and held it in contempt. I pleaded with the Health Secretary to be present at this meeting that RCPsyc called for even if allowed to be in the background with no speaking rights. I wanted to hear what was going to be said. Every surgeon who performs high risk surgery, does so with their skill and training and sometimes things may go wrong but they don’t do operations badly and think they’re not going to be held accountable. (I declare an interest here, my brother is a Trauma & Orthopaedic Surgeon, he operates knowing full well he will be fully accountable should he make a mistake during surgery. He and his friends in various branches of high risk surgery find the idea of being unaccountable laughable. They also point out, there no shortage of high risk surgeons. Never hear the Royal College of Surgeons say, there will be a shortage of surgeons as their job is high risk) For a branch of medicine to think they can be professionally unaccountable is phenomenally ignorant. Psychiatry is the only branch of medicine where if you are lazy and can’t be bothered doing your job properly your patient may end up harming themselves or members of the public. To think you can work without accountability is absurd. Just the announcement by Lade Smith’ “I’m aware of the concerns across the membership regarding the increasing risk psychiatrists are facing in terms of culpability and accountability, particularly in the context of patient safety incidents. We share these concerns”. I’ll bow to the public to decide their views and what they think about the statement. I know what I think…….

So you’re consultant incharge of service but think you’re not in charge of service and not to blame for discharging VC!?! You think clinician in charge is a CPN or AMPH ! You don’t do discharge letters to GP, clinicians do ( but that’s not you) ! You take no notes at MDTs. @drsanjoykumar I’ve been saying this for many years- How many attempts qualifies reasonable in law? Professional integrity for me suggests you should visit at least 3 times at different times of day. Most of public sector staff enjoy the pay check with minimal work! Repulsive !

Today’s main evidence was from Claudia Birtles who was irreverent and unbelievably devoid of an empathy. She is VC‘s Care Coordinator now known as lead professional. In charge of case since first diagnosis in Nottingham of May 2020. She’s in charge of looking after his care plan, attending multidisciplinary team meetings and see that things are going according to the clinical plan. Examination of the notes today reveal the MDT’s seem like a chat, there’s no agenda, no template, no organisation and there seems to be no structure to these meetings. There were no minutes taken and hence no documentation. As you and I both know if something isn’t written in the medical notes it basically didn’t happen. Throughout the whole of evidence there was a feeling of lethargy, no curiosity, not challenging the Psychiatrists with their flawed management who themselves were of the poorest professional standard that I have come across. Basically there is no plan in place. It seems like a clinically stumbling stroll. The care coordinator is meant to know their patients really well yet knowing lack of concordance, there is no suggestion of a community treatment order (CTO) after Sec-3. Just repeated recording of lack of concordance with medication with no plan to pursue. He’s not taking medication and hence section after section repeated x4 with absolutely no improvement in management. What does improve is Birtles self management where she puts in the notes that VC is not to be visited singularly because she felt unsafe and the irony here that really upset me is that she didn’t give a second thought to the safety of much younger students. She never contacted University to alert of risks. Today’s evidence was particularly upsetting because at the end of every sentence that this Care Coordinator came up with she could have insisted on a different therapeutic direction like depot, but didn’t insist. Devoid of the vision of what might happen. Had there been a different course of Mx, depot, assertive outreach, any change of clinical direction ,VC I’m convinced would’ve missed our children. Claudia Birtles treated the whole 2 year management like some paper exercise with no positive intervention by herself. Selfish, lazy unprofessional but more importantly to me she had the opportunity to change VCs course and she didn’t. As for her totally inappropriate smiles and laughter, showed a total lack of empathy for the bereaved or respect for the Inquiry process. @bbcemt @BBCNews @GBNEWS @nottslive @nmcnews @wesstreeting

Now let me share my experience of the NHSE/Theemis commissioned report. I asked a basic question when fighting for an Inquiry: was it 4 Consultants that failed at 4 Sectioning events or 1 consultant 4 times. I have never accepted the NHSE/Theemis report and now you see why…….VC actually had many MHA assessments. He was seen by multiple consultants many more than the number I expected and by tens of other grade doctors and mental health nurses, team leaders, ward managers ….and they ALL FAILED. Nottinghamshire Trust failed at every level, every grade. A corporate failure And there’s more to come today…. @CareQualityComm @wesstreeting

No understanding by any of the Psychiatrists so far about the “least restrictive option” in the context of public and patient safety. With regards to the Psychiatrists’ evidence to the Inquiry…..just when I think things can’t get any more absurd……..they do. I am actually sitting in Mary Ward House everyday pulling my hair out.
