My mum after nursing for 40yrs has been told that her ENB997/998 is not sufficient to meet the requirements of being a mentor to student nurses… is that right? I looked at guidelines and I don’t think that advice is correct… can anyone help? @alisonleary1

I’m not sure who needs to hear this, but denying patients discharge paperwork and medications because they’re self discharging is cruel and harmful. They might be acting against advice, but we should still make their discharge as safe as possible.

@StN_Freda The other issue IMO is there are also experienced nurses changing specialties/ areas of work / wanting to down grade as close to retirement etc so NQNs are up against those who come with all the experiences and skills already acquired. It’s tough for a NQN going against that.

Being a Mum and mature student, I chose nursing over SW or OT as I needed a strong job market. After acing my 1st interview with positive feedback, I was told I lacked experience in the specialty, despite 3 of my 8 placements being in that area. The system feels broken for NQNs.

Palliative medicine not just death and dying - examples from practice: Anti-depressant cross titration, ketamine intravenous infusions, tranexamic acid infusion, methadone cross titration, central post stroke pain management, sativex for spasticity, interventional cancer pain techniques, hypoadrenalism, chronic non-cancer pain, dystonia, MND and Parkinson’s symptom control, rotigotine patch conversions, needle phobia, octreotide infusion conversion to depot lanreotide, CBD drug interactions, diabetic control on steroids, chemotherapy induced peripheral neuropathy, clondine bridging for pain crisis, prescribing in substance misuse, paracentesis, scut betablocker for symptomatic SVT, delirium, managing sweating or fever secondary to cancers, home oxygen ordering, managing itch/dysaesthesia, IV bisphosphonates, acupuncture, transmucosal drug administration, spinal cord compression management, prophylactic fixation of long bone metastases, vertebroplasty, cordotomy, neuroleptic malignant syndrome, syringe driver compatibility, dysgeusia resolved with zinc, creon shortages…… that’s just some of the medical side… the care and close interactions with patients are the really enjoyable parts. Palliative care as a medical career will keep you up to date and innovative as a doctor if you are in the right setting. Yes we look after people who are dying but being “made palliative” is an escalation in care and attention if truly done well. #careerinpalliativecare #newdoctors

@AlbionCatA @NurseStandard I think most nurses can decide if a patient can have simple OTC remedies without requiring a doctor’s prescription. I know there will always be exceptions but I’m thinking simple things that nurses use regularly that we can assess for.

@NurseStandard The problem is that in order to prescribe you should do advance clinical assessment at level 7, this isn’t appropriate at band 5/ newly qualified. Aim then should be preceptorship and consolidation. Also prescribing isn’t in B5 JD, nor should it be.
