Over a median of 18 years, the prostate cancer–specific survival rate remained high at 94% at 25 years, with especially favorable outcomes in very-low-risk patients (99%). However, the risk of treatment or disease progression increased over time, with only 38% remaining treatment-free at 22 years. The findings affirm the long-term safety of AS for appropriately selected men but underscore the need for lifelong monitoring and patient counseling, as there was no standardized AS protocol applied.
