#OTILT HIV treatment regimens with INSTIs appear to cause weight gain, however this may just be due to the patients in studies switching from TDF to TAF📈 In one US study, switching from TDF to TAF lead to a significant weight gain regardless of the INSTI the pt was on🧐

#OTILT in the case of culture negative septic arthritis, it is prudent to consider mycoplasma as the causative agent🕵️‍♀️ although rare, it is most common in the immunocompromised😷 and should be investigated is there are repeat negative cultures and poor response to empiric abx💊

#OTILT In c. diff negative patients, klebsiella oxytoca is a probable cause of antibiotic-associated hemorrhagic collitis (AAHC)🤒. It produces a cytotoxin that induces epithelial cell death and increases the risk of AAHC in patients on amoxicillin💊

#OTILT chorioamnionitis is an infection of the placenta of a pregnant person🤰, most often due to the ascension of bacteria from the vagina. It often presents as fundal tenderness and tachycardia🫀. Treatment includes ampicillin +ngentamicin +/- clindamycin until delivery👶

#OTILT what to do if Bactrim is not an option for PJP? Aerosolized pentamidine is an option but is not recommended due to frequent relapse. IV pentamidine is efficacious but comes with many AEs🤕. Best second line in patients without G6PD-deficiency is clindamycin+primaquine✅

#OTLIT fluoroquinolones should be used cautiously in patients with vascular disease 🫀 the class has been associated with significantly increased risk of arrhythmia, MI and CV mortality📈 the worst offender is moxifloxacin and the least bad option is ciprofloxacin
