#OTILT In 1998 the LYMErix 💉was approved that used recombinant outer-surface protein OspA. LYMErix showed a 76% decrease in lyme after 3 doses (@ month 0, 1, and 12). It was pulled off the market by the manufacturer after increased reports of arthritis and musculoskeletal sx.

#OTILT Clostridium difficile is typically flora that can be found the GI tract. However, a cx positive for c. diff does not necessarily mean we need to treat! Colonization is when it is detected but the pt is asymptomatic. Infection requires a positive TOXIN +symptoms.

#OTILT Rifampin is our most effective drug to treat TB, but resistance can limit use. Monoresistance is uncommon, so if a PCR shows rifampin resistance, it’s likely also resistant to isoniazid (AKA MDR).
MDR TB tx: moxi/levoflox +bedaquiline+linezolid+/- clofazimine+/-cycloserine

#OTILT Dapto+beta-lactams are used for synergy against enterococcus. But what if there's a BL allergy?! In vitro studies show limited synergy with dapto + gentamicin, rifampin, or tigecycline, however it was less consistent than dapto+ampicillin and in vivo application is unknown

#OTILT in HCT tx, genotyping is used to determine most appropriate therapy. Mavyret and Epclusa are pangenotypic, but Harvoni only covers GT 1,4, 5, 6. Vosevi is a 3 drug combo (sofosbuvir/velpatasvir/voxilapravir) that can be used for difficult to treat pts w/ GT 1-4.

#OTILT Pericarditis is a complication of viral or bacterial infections. Recurrent pericarditis may occur 4-6wks after acute presentation. Tx w/ corticosteroids has been used previously, but recent studies found CCS use is associated with prolonged dz and higher recurrence risk.

#OTILT Infective endocarditis is commonly treated with IV ABX for a course of 6 weeks, often requiring a PICC placement for outpatient IV ABX. Switching to oral ABX for outpt tx in clinically stable pts was found to be non-inferior in safety+efficacy compared to a full IV course.
