The real problem here is that police have very limited to zero training in these areas. They qualify once a year with a handful of rounds into a piece of paper at 20 meters and think it's sufficient.
Recruiting and performance standards across the country are abysmally low. No one should expect anything better than panicked, amateur performances when the priority of the police is pandering to diversity quotas, feelings and investigating mean tweets.
Quality was sacrificed on the altar of woke politics so this is what you get.

A very difficult situation . The officer sees her partner killed next to her, while taking cover from armed shooter less than 25 meters away and can't see him.
Man approaches her from the same direction and for all she knew, he was the suspect. She flinched and shot him.
