@Boghuma And colchicine does not cure COVID--it mitigates symptoms and prevents progression, a useful adjunct.

@Boghuma Take the X away from colchicine! RECOVERY was badly designed--testing colchicine in the setting of dex was sloppy. #COLCORONA is well designed-n almost 4500-and shows benefit--don’t throw baby out with bathwater.

#ColCorona published! (@LancetRespiriMed). For PCR+ #COVID19 outpatients, 0.5 mg daily colchicine reduced hospitalization/pneumonia. Among select populations, NNT was around 30. For the right patients, colchicine is cheap, safe and could save resources and potentially lives.

ColCorona data are online. Still under peer review though! Consideration for early intervention with colchicine if COVID PCR positive outpt and high-risk. Agree, wish gone to the planned n of 6000. MHI had a difficult decision to make during this challenging time. @MHPillingerMD

The large colchicine RCT for covid is preprint published. It shows some consistent directional benefit for major outcomes medrxiv.org/content/10.110… Since this is a pill, it's safe, cheap & an early intervention (as tested here, before hospital) these findings are very encouraging

@ashira_md @ALGaffoRheumMD @ashira_md, if you view the curves, inflammation starts early post infection but continues to accelerate even as viral load declines. I see it like a runaway train--you want to get the blocks under the wheels before it even leaves the station. Hard to stop a runaway train!

@MHPillingerMD @ALGaffoRheumMD I wondered about treating early too. Even the SARS CoV-2 MAB treatments didn’t work in hospitalized patients, but did in outpatients. I think these therapies are much more effective in general before someone is sick enough for admission

Smart idea @ashira_md! Our past research shows colchicine inhibits neutrophil-platelet but not platelet-platelet aggregation. But we hypothesize that inflammasome inhibition may be at least equally important here.
