@AbbasiMuqtadir Agree of course to start ATRA at first suspicion of APL. Note pathogenesis of the coagulopathy is complicated and includes DIC, fibrinolysis, and proteolysis. The fibrinogen can occasionally even be normal at diagnosis!

✅ Correct Answer: C. Acute Promyelocytic Leukemia (APL) 🧠 Explanation This flow cytometry pattern is classic for APL: 🔑 Key clues: CD33 bright Strong MPO positivity CD34 negative HLA-DR negative 📌 This immunophenotype should immediately raise suspicion for APL (t(15;17), PML-RARA). 🚨 Clinical pearl: If APL is suspected on flow → start ATRA immediately, even before cytogenetic confirmation, due to high risk of fatal DIC.

A true story: a friend’s dog developed lethargy and cough, vet took CXR which showed pneumonia. 2 days later friend developed lethargy and cough, internist did CXR showed pneumonia treated w Z-pak. Was this a case of CAP (Canine Acquired Pneumonia)?

@MartinTallman That’s quite a coincidence 😬 CAP usually refers to humans, so it’s probably not the same thing. Still, pretty scary timing @MartinTallman
