You’re intubating a patient who has bad pneumonia and likely ARDS. The patient is on BIPAP and is being pre oxygenated with that. Sat is 94% despite 💯 % fio2. You position appropriately and do all steps for high first pass success. You get a view but the tube FALLS ON THE FLOOR

@emily_fri I’ve read a PFT or two where the lab inadvertently entered “70 cm” for height instead of 70 in. Made for some very impressive % predicted values 😂

@MatthewMartMD I'll check into those other equations. I used the height/weight/sex/age listed at the top of the test so I assume they're the same and can't figure out why it's SO different (like, 2900 vs 1300).

Do you have a favorite reference equation for predicted VO2 max? Do you know what equation your lab uses?

@emily_fri It’s the best CPET book! Your lab may be using the FRIEND registry equations (or European equations). Most metabolic cart software has preset equation options, so you could have your team check the settings in the software. And they can double check that height/wt are correct.

@MatthewMartMD That’s the book that I have/love and the equation I checked our lab’s predicted values with! I’m not sure if we’re using some out there equation or if something is messed up with inputs.

Over 5 million patients are admitted each year to ICUs in the US. With advances in modern medicine, ~70% survive. Many survivors experience Post Intensive Care Syndrome (PICS), a collection of new or worsening impairments in the cognitive, physical, or mental health realm. These impairments are often long-term, resulting in a difficult recovery journey and leaving patients feeling isolated and misunderstood. Join us for the inaugural National #Walk4PICS Day by walking any appropriate distance Saturday, September 30. icudelirium.org/international-… #MedTwitter #nursetwitter #ICUrehab

@HeidiEngel4 @WesElyMD @DaytonICU Thanks for the good question Dr. Engel! We plan on collecting sedative exposure data, so hopefully we can do some secondary analyses looking at associations.

Any chance the mitochondrial toxin propofol used to sedate #ICU patients on mechanical ventilators can be incorporated into this? Confounding variable? Source of long term #PICS? Thank you @WesElyMD & @MatthewMartMD for amazing work @DaytonICU

Science: PICS & LongCOVID 1st Generation College Student turned ICU Dr. @MatthewMartMD just received a PERFECT 10 on his @NIH K23 to study muscle & nerve disease that we believe is mitochondrial in nature. 🤔 #PEM? I’m thankful to mentor him & see his devotion to discovery.

@emily_fri This really depends on your academic track and your promotion pathway. It could be research, educational endeavors, QI, admin, etc. I balance office time and work from home different each week depending on what priorities (kids, papers/grants, education) are most pressing. YMMV

Academic docs—how do you handle your Not On Clinical Service Weekday time? Do you go into your office every non-service day for a set amount of time? Do you work from home? Do you do non-work things? Is there a right answer?
