Requests for “unvaccinated” blood are causing delays, clinical complications, and resource waste in hospitals.
A small but growing number of patients and families are refusing standard blood transfusions from the community blood supply because of concerns about donations from individuals vaccinated against COVID-19. This trend is creating operational challenges and, in some cases, putting patients at increased risk.
Researchers at Vanderbilt University Medical Center documented 15 such requests between January 2024 and December 2025. During this period, the hospital’s blood bank processed 144,856 blood product units, with directed donations accounting for only 0.03% of the total. In all 15 cases, patients or their caregivers specifically requested directed donations from donors they believed were unvaccinated.
Thirteen of the 15 patients received at least some of the requested directed units. However, the insistence on unvaccinated blood led to documented issues: treatment delays, postponed or canceled surgeries (especially in pediatric cases), and inefficient use of resources. In two cases, patients experienced clinical deterioration while waiting, including significant anemia and hemodynamic instability. Several directed units went unused or expired.
There is no scientific evidence that blood from COVID-19 vaccinated donors is less safe than blood from unvaccinated donors. Major organizations such as the FDA and AABB affirm that the screened community blood supply remains safe and effective.
Directed donations, by contrast, often come from first-time or one-time donors and can carry a higher risk of undetected infections compared to the highly regulated, repeat-donor community supply. Historically, directed donations were used in specific situations (such as rare blood types), but experts warn that the current surge, driven largely by concerns over mRNA vaccines, is medically unjustified and complicates care.
While some U.S. regions have seen legislative efforts to guarantee access to “unvaccinated” blood, many countries (including the UK and Australia) strictly limit such requests. Medical professionals emphasize that prioritizing unproven preferences over evidence-based transfusion protocols can endanger patients and strain hospital resources.
