During the ARDD 2025 conference the researcher @NirBarzilaiMD presented an updated version of a ranked list of FDA-approved compounds with longevity potential. One of his goals with this list is to help guide research efforts and financial investments in the field. One thing to keep in mind is that even if these compounds have longevity potential they will likely never result in radical life extension as single interventions. Perhaps they can provide an extra decade or so of life. Regardless, I believe there is a benefit in pushing research forward on these promising compounds because the natural next step is to explore combinational therapies based on them. In his paper Barzilai proposes that the four top compounds the field should focus on are SGLT2 inhibitors, Metformin, Bisphosphonates and GLP-1 receptor agonists. It’s an interesting prioritization and I almost agree with him. I also like the table overview but I would like to improve the ranking of preclinical lifespan studies, specifically regarding the ITP (Intervention Testing Program) studies in mice. The ITP is considered the gold standard for mouse lifespan studies. I have adjusted the ranking slightly because as it stands today the lifespan data for Rapamycin outperforms all other compounds tested in the ITP. The ranking could be further improved by including preclinical studies in multiple species, even there Rapamycin outperforms most compounds. With this minor adjustment to the preclinical lifespan ranking, Rapamycin joins the top compounds the field should focus on. There are ongoing clinical studies in humans and two major ones are set to begin soon (see my previous post regarding the ARPA-H PROSPR funding). My guess is that once we have data from these trials then Rapamycin will be the top candidate on this list. So it is just a matter of time but it is frustrating that progress moves so slowly. Two major funding milestones have been reached in the longevity field! These awards will support critical research into mTOR inhibitors and their ability to modulate aging in humans. 1) The VITAL-H trial is the first Phase 3 human clinical trial targeting aging in generally healthy adults. It was awarded $38 million from the ARPA-H PROSPR program. In this trial, researchers at the Barshop Institute will test whether repurposing the FDA-approved medications Rapamycin (mTOR inhibitor), Dapagliflozin (SGLT2 inhibitor), and Semaglutide (GLP-1 receptor agonist) can delay age-related functional decline in people aged 60 to 65. They will evaluate these drugs' potential beneficial effects on cognition, mobility, and vitality. The trial aims to move beyond treating single diseases to preserving overall health. 2) In parallel, Cambrian Biopharma has secured a $30.8 million PROSPR award to advance its next generation of mTOR inhibitors. Their leading compound, TOR-101, is a highly selective mTORC1 inhibitor with a strong safety profile when tested in non-human primates. It’s now time to conduct human clinical trials and evaluate if it can improve longevity in healthy humans. nature.com/articles/s4158… The first FDA approved clinical trial testing rejuvenation with partial cellular reprogramming factors is about to start! This is a huge moment for the longevity field. Partial cellular reprogramming has the potential to not just slow biological aging, but reverse aspects of age-related decline. This is not theory; it has been shown in preclinical studies, including data from Life Bioscience's founder, David Sinclair's lab, demonstrating restoration of vision in partially blind mice. This 5 year clinical trial will test whether pulsed delivery of three cellular reprogramming factors (O, S, K) can restore vision in individuals with forms of optic neuropathy and glaucoma -- common drivers of vision loss. Keep in mind, this technology relies on a form of gene therapy: cost, scalability, and accessibility are major bottlenecks. Long-term safety in humans is still a major outstanding question. The time horizon for this particular technology to reach the masses (if it succeeds in restoring vision) is 1-2 decades at a minimum. A small price to pay for the potential massive impact of rejuvenation. Even more reason for optimism. @OMIPALRLL and @ReprogLab are working hard in parallel to develop and validate chemical reprogramming factors that capture the rejuvenative impact with small molecules, peptides, familiar factors that can greatly reduce cost, improve safety profile, and accelerate time to market and accessibility. Not just for restoring vision -- but age-related diseases that impact the brain, immune system, heart, and muscles. Potentially even prevention. 2026 has started with a breakneck pace, and the future of longevity is bright! The fundamental strategy of today: 1) Track and support labs/companies driving the breakthrough science 2) Stay healthy enough for long enough to engage with the rejuvenative technologies already in the pipeline.

The ITP (Intervention Testing Program) is the gold standard for testing longevity interventions in mice. We are currently creating an ITP proposal for the FDA-approved compound Doxycycline. This compound showed truly impressive lifespan effects in roundworms (PubMed: 39853804). What we know so far: 1️⃣ Roundworms: ~95% median lifespan extension (PubMed: 39853804) 2️⃣ Flies: 34% life extension in males and 24% in females (PubMed: 39853804) 3️⃣ Progerian mice: ~16% extension in median lifespan (PubMed: 38686927) We are very interested to see if this translates to UM-HET3 mice. If this is the case, the next step is exploring synergistic effects—for example, if the 109% median lifespan extension observed when Doxycycline was combined with the dual PI3K/mTOR inhibitor Omipalisib also translates to other species (PubMed: 39853804). We are in the proposal stage and all feedback is welcome! Let’s push the boundaries of human longevity!

This is a monthly update of the Rapamycin Longevity Lab (@omipalRLL) Pipeline on @pumpdotscience. Our core portfolio is built around the highly evolutionary conserved mTOR pathway which is widely viewed as the gold standard in the longevity field. Our mission at Rapamycin Longevity Lab is to explore opportunities to leverage this pathway and push the boundaries of human lifespan in order to take steps closer to radical life extension. Here’s the latest update on our premium portfolio: 1⃣ Doxycycline ($DOCS): An FDA-approved antibiotic with a strong safety profile. We took a “King of the Pill” position as the best-performing lifespan compound in worms on Pump Science, with more than 80% median lifespan extension. Another lab has observed 93%. Some suggest the large increase may be due to the effect of killing bacteria and by that mimic calorie restriction. But another lab saw impressive results using both live and dead bacteria which indicate other mechanisms drive benefits beyond calorie restriction, including anti-inflammatory properties and enhancing mitochondrial health. 2⃣ Chemical Partial Reprogramming ($TREX): In partnership with Reprogramming Lab (@reprogLab) we launched this token. It’s a cutting-edge longevity intervention with big potential if it works to reverse aging. Worm data showed 13% life extension but another lab has seen 42%. We are analyzing the data to see what the next step forward will be. 3⃣ Omipalisib ($OMIPAL): Two years ago we discovered that the dual PI3K/mTOR inhibitor Omipalisib extended worm lifespan with a maximum median increase of 63%. That is more than double Rapamycin’s lifespan effects in the literature. Fly experiments showed 6% life span extension. We are now trying a different dose protocol to see if we can increase the lifespan even more. Mice experiments are also starting soon. 4⃣ Omipalisib + Doxycycline ($OMIDOCS): This combination has shown a remarkable 109% median lifespan increase in worms by another lab. In our own experiments we observed a 56% extension which secured its place as the next best-performing intervention on Pump Science. Fly experiments are set to begin once the ongoing Omipalisib fly experiments are completed. 5⃣ Rapamycin ($RAP): This is a well-known mTOR inhibitor with robust longevity data across species. Rapamycin Longevity Lab took full developer ownership of the token after Eterna Labs closed. The latest fly data from our experiments showed around 18% median lifespan extension. The best effect observed in the scientific literature is 38%. Mice experiments are starting soon. 6⃣ Hair health startup ($DOCS, $RAP, $RAPADOCS): We have set up a startup for entering the multi-billion market around hair health. Focus is on reducing hair loss, preventing grey hair and improving growth. We are exploring opportunities to develop products around Doxycycline ($DOCS), Rapamycin ($RAP) and Rapamycin + Doxycycline ($RAPADOCS).
