Many frail older people are being rapidly discharged to interim placements eg hastily commissioned beds in care homes with limited therapy or social work input. And often distant from own 1ry care team.. what can hospital teams do to mitigate risks? @GeriSoc @adamgordon1978

The last 2 weeks = an experiment in how low you can take NHS staffing without terrible things happening. Like a game of Staff Jenga. Things have not been a disaster because everyone is working their butts off, missing breaks, doing the work of 3 ppl. Amazing - but unsustainable
