🔴SPEAR - DAY 1🔴 Today marked the launch of our brand new 3-day ATACC SPEAR course 🚀 This programme goes beyond current ALS - diving deep into physiologically guided resuscitation and pushing clinical thinking to the next level. Day 1 set the foundation: From understanding the physiology of cardiac arrest to optimising ventilation, chest compressions, and defibrillation. Plus a deep dive into the 2025 Resuscitation Guidelines and the evidence behind them. Our teams hit the ground running with the ‘Repeat’ scenario - building teamwork, confidence, and kit familiarisation - before progressing through a series of scenarios reinforcing the morning’s theory. The afternoon introduces the anatomy and practical aspects of endovascular access for arterial monitoring and resuscitation. Followed by more immersive scenarios integrating advanced physiological monitoring with ALS skills. 🔴SPEAR - DAY 2🔴 The complexity stepped up today with a deep dive into cardiac and cardiovascular physiology - focusing on what truly improves the chances of ROSC and positive neurological outcomes. Morning scenarios pushed immersive simulation further, challenging teams to manage cardiac arrest through physiology-led decision making, monitoring, and targeted interventions. In the afternoon, we made the most of the sunshine in our TAG outdoor space - exploring right heart & pulmonary physiology, plus aortic physiology, comparing full occlusion, partial occlusion, and adrenaline. We wrapped up with four high-intensity scenarios: resus arrests, thrombolysis, REBOA, and prehospital cardiac arrest across varied aetiologies. The day ended with an al fresco EVTM REBOA masterclass from Halden 👏 The progression has been clear - teams are becoming sharper, more coordinated, and increasingly confident from fundamentals through to advanced endovascular techniques. Their CRM and leadership gets better by the hour…but as complexity increases, so does the challenge of managing and positioning kit and equipment in a logical way - there’s still plenty to learn, and tomorrow will bring more cardiac arrests and even more special circumstances. Well that’s a wrap and what a fantastic 3 days of advanced cardiac resuscitation with some amazing people from around the world! We started this morning with a summary version of ‘Resuscitator’ to see what our delegates would do, after several days of cutting edge research, evidence and practice, together with a paired final practice of ultrasound guided needle techniques. Then we were back onto more scenarios with challenging clinical cases such as complex overdose and unstable post-ROSC critical care, while outside the situations and scenarios became even more challenging but every step of the way the candidates upped their game and delivered. Today was about fine tuning and mastery of skills before the final afternoon of practical REBOA scenarios and the essentials of ECPR. It has been a truly whirlwind 3 days, but for anyone who wants to take their resuscitation to the next level and to mix with like minded people to discuss the latest principles, research and practice then SPEAR is for you. I am officially retired from @UWMedFlight . My final flight as a HEMS physician was today 4/11/26 It started in 1989. Over the last 36 years I have transported 4000+ patients with 3 HEMS programs: Univ of Chicago 1989-92 OSF St Anthonys 1995-200 Univ of Wisconsin Med Flight 1992-2026 I have worked in 10 helicopter models: SA 365 Dauphin N, N1, N2 AS 355 Twinstar Bell 222 Agusta 109 Power Agusta 109 Mk2 BK 117 EC 135 EC 145 What a long, strange trip its been!

Another outstanding week of training at ATACC Group. We’ve just completed our Tactical Medical Planning Course, followed immediately by our CBRNE Medical Response Course - a powerful combination that bridges strategy, planning, and operational delivery. Our Tactical Medical Planning framework continues to prove its value. It provides a structured, practical approach to developing medical support plans that are adaptable, threat-informed, and operationally grounded. Delegates leave with a clear methodology they can take back to their organisations - enhancing capability, governance, and real-world readiness. Taking that foundation straight into our CBRNE Medical Response Course allows participants to apply planning principles in high-fidelity, immersive simulation. This course is specifically designed to complement existing organisational CBRNE training by introducing complex, realistic scenarios rooted in criminality and terrorism within the civilian environment. This is training for reality. Beyond the core foundations of CBRNE response, we focus heavily on: • Decision making under pressure • Critical thinking in dynamic environments • Tactical awareness and threat-informed practice • Multi-agency interoperability • Leadership in uncertainty Our approach ensures clinicians and operational teams are not just technically competent - but operationally capable. @padoscan Completely agree. We need a far simpler surrogate. However, the bigger message from the trial is that our routine practice may well be significantly ‘off the mark’ and we should be prepared to adapt our Resus, even if just based on EtCO2 rise.
