Great to kick off @crfheart #CTO2026 this morning with a case-based CT-guided PCI crash course alongside @ColletCarlos @jcspratt. Excellent discussions & hands-on training, including use of the PCI Navigator. We are at an inflection point in coronary pre-procedural planning and triage. Not all severe coronary disease presents with chest pain. This live case highlights how advanced coronary pathology can remain clinically silent and why imaging-led decision-making matters. Performed by @jcspratt & @Ganeev at St George's University Hospitals NHS Foundation Trust. The case: A 62-year-old patient presenting with a large stroke underwent echocardiography to investigate a potential cardiac cause. The echo revealed a reduced LVEF with regional wall motion abnormalities, raising suspicion of significant coronary artery disease. A subsequent CT coronary angiogram confirmed severe coronary disease. The patient was asymptomatic, likely because residual post-stroke weakness limited exertion and masked angina. Invasive coronary angiography later confirmed severe double-vessel disease. PCI to the LCx was successfully performed, with a planned staged LAD CTO PCI delivered as a live case. Key takeaway: Absence of symptoms does not equal absence of disease. Non-invasive imaging can be pivotal in uncovering high-risk coronary anatomy and guiding proactive, staged revascularisation strategies, even in patients who appear clinically stable. @ShockwaveIVL @AbbottCardio
