🧠 Claude Code has been taking notes about you for months. Silently. Without asking your permission. And that notebook is called MEMORY.md. 📄 Introduced in Claude Code version 2.1.59 as part of the auto-memory system, MEMORY.md is the index file where the agent stores what it learns from you session after session: build commands, architecture decisions, debugging patterns, preferences detected in your corrections. It lives at ~/.claude/projects//memory/ and loads automatically at the start of every session (only the first 200 lines or 25KB; the rest goes into topic files loaded on demand). 🫵🏻 Best practices that make the difference: 1️⃣ Use it as your conscious session close. Before hitting /clear, tell Claude: "write down in MEMORY.md the current state, what's pending, and the decisions made." That way you close with clean context and start the next task without dragging noise along. This is THE move to get the most out of MEMORY.md. 2️⃣ Audit it periodically with /memory. It's plain Markdown. If Claude learned something wrong, it will propagate that mistake session after session until you fix it by hand. 3️⃣ Keep it under 200 lines. Anything over that doesn't load. Claude automatically splits the detail into topic files (debugging.md, api-conventions.md…) that get read on demand. 4️⃣ Remember it's machine-local. It doesn't sync across machines or cloud environments. Each computer has its own memory. 🤯 And now the uncomfortable part: context degradation is NOT an opinion, it's math. The "Lost in the Middle" paper (Liu et al., 2023) showed that LLMs lose precision with information placed in the middle of long contexts. Translated to daily practice: your agent starts duplicating functions it already wrote, forgets conventions established at the start, hallucinates file paths that don't exist. It's not random. It's degraded attention. ⚡ That's why /clear is not optional: it's the most important tool you have. The ecosystem's pragmatic rule is clear: if you've corrected Claude twice on the same problem, the context is already contaminated with failed attempts. Hit /clear and start over with a better prompt that incorporates what you learned. A clean session with a good prompt almost always outperforms a long session full of patches. 🔑 The winning flow is simple: atomize tasks (one session = one task), close each one by writing the relevant parts to MEMORY.md, /clear, next. This isn't optional discipline. It's the difference between a reliable teammate and an amnesiac intern that makes you waste hours. ✨ Coding with AI isn't copy and paste. It's understanding where context lives, how it degrades, and when to clean it. Those who get it, multiply. The rest keep fighting with an agent "that doesn't understand them."
