Today, the Government has published its long-awaited National Plan to End Homelessness. We’re delighted to see a firm commitment to ending the unacceptable practice of discharging people from hospital to the street. @Pathwayuk has long called for this vital and overdue change.

Doctors are the experts on health, and what helps and harms our health. It’s therefore so powerful to see them standing up for the dangers faced by people experiencing homelessness and calling for action in the Government’s forthcoming homelessness strategy. We support the call.

In 2024/25, the specialist homelessness teams: ✅Helped 4,778 patients experiencing homelessness, a 41% increase on 23/24 ✅Reduced returns to rough sleeping by 62% and to sofa surfing by 33% ✅Registered 70% of patients with no prior access to primary care with accessible GPs

With funding re-established, we’re thrilled that HIT’s life changing work is continuing. Prioritising safe hospital discharge, the multidisciplinary team's highly skilled, person-centred approach has reduced the number of patients returning to rough sleeping by 60%.
