@kdjhaveri If we hope to improve outcomes for our patients, we need to have a robust cadre of outpatient nephrologists. If having nephrohospitalists allows for that, then perhaps it is a good thing. But, longitudinal relationships with patients, IMO, are much more rewarding.

@DrDanMO Not expensive now. 30d supply of Cinacalcet is $5.39 at Costco with GoodRx coupon. Have patient take 30mg/day at dinner and see if nausea persists.

Due to the high cost, cinacalcet is commonly administered 3x/week during hemodialysis by the HD unit (which is paying for the drug). Today, I have another patient who gets nausea and vomiting after every dose (90 mg/day)....and don't say use etelcalcitide.

@AmyPearlman1 CoVID has changed this. Patients are encouraged not to come to office if they feel sick. We charge if no notice. Collecting is difficult. I’ve offered to switch to telemedicine on the fly. Advance plan for telemedicine when you know your patient has transport issues.

Several no shows to my clinic so far and it’s only ten am. Does your clinic charge for same day cancellations?

@Teknofiliac @eric_weinhandl Is this data accurate? We have 90-100 PD and another 50 HHD patients in our program and it looks like there are no programs of that size per this graph.
