@MTPapaD @DrHarryThomas Very true. Over 2/3 patients diagnosed with bile acid diarrhea responded when switched to another sequestrant. So testing before empiric trial is very important

@MTPapaD @DCharabaty Absolutely. Lots of meds makes timing very challenging. Fxr targeted medications are on the pipeline that should resolve this issue but we need to keep testing before trialing empiric therapy.

@MTPapaD Definitely agree with starting empirically with those > 10 cm TI resection. Empiric tx for many patients can often lead to mixed results from side effects to non response. Simpler test coming out very very soon!

@ebtapper @DrReddyGI @AmCollegeGastro @MayoClinicGIHep @LizzieAbyMD The problem is that most patients undergo extensive dx testing and then may be empirically started on sequestrant tx. With a cheaper dx test up front for BAD, we can decrease dx costs and provide targeted tx.

The inputs are how bad the patient feels, the prob of relief with tx, the sens/spec/cost of tests. Is a partially effective 1st line tx up front more cost-effective than testing first? What does the pt value?
