@HuwJakeway @Cerithg 100% behind the decision Huw. Training for competent & responsible front line staff is essential. This isn’t a day trip by irresponsible tourists interacting with the public! These are professional life savers. rehearsal of emergency procedure is critical.

With a statutory duty to respond to water rescue and flooding. I really thought you’d understand that it is essential that our crews maintain their operational compliance and competence!!
Perhaps @Cerithg has a view on operational competence?

@ACRWare @isabelholc @ComplexWales @snowded @BevanCommission @SionCharles @SwanseaUni The book is just a recent interest in thinking slow by using informants to talk through what they are doing and why they do it. It fits with heirsrchical task analysis and safety. Happy to talk and refine everything. I enjoy hearing your views and what you’ve been up to. BW Nick

@ProfNickRich @isabelholc @ComplexWales @snowded @BevanCommission @SionCharles @SwanseaUni Worth me reading the book you showed on Monday? Or do you have a better recommendation? Didn't tell you Monday but your talk on Lean was a great refresher & lesson for ordered system. Thanks again & would love to show you some work in primary care that touches on all that

I'm listening to @ProfNickRich and shortly @snowded . Order & complexity. This is still ringing in my ears as Ive heard it for the last 4 years. 'What if Russell Ackoff did a Ted talk' youtu.be/HbyLgzge63A HT @isabelholc 'Doing the wrong thing right'

@ACRWare @isabelholc @ComplexWales @snowded @BevanCommission @SionCharles @SwanseaUni Many defences are used - team work, peer pressure, system continuous learning,SOPs, failure correction and a bias for prevention of system factors that lead to errors and lapses. Heightened honesty and low retribution for honest mistakes are features of the just culture (glue)

@ProfNickRich @isabelholc @ComplexWales @snowded @BevanCommission @SionCharles @SwanseaUni I understood (& might be wrong) that Safety 2 looked for solutions that made problems irrelevant preventing rule breaking by granting permission, rather than a 'safety swarm'. @snowded 's landscape maps shows the contrast between rule breaking and getting the job done in the NHS
Cardiff, Wales 🇬🇧 English

@isabelholc @ComplexWales @ACRWare @snowded @BevanCommission @SionCharles @SwanseaUni True. People forget that discipline is still needed for blatant rule violations and prolonged abuse of protocols. Safety II proposes a just culture will reduce blatant rule breaking because many team members will confront offenders quickly.
English

@ComplexWales @ACRWare @ProfNickRich @snowded @BevanCommission @SionCharles @SwanseaUni We all fear of failure especially in healthcare, learn to learn fromintelligent failure is important but you don't know to focus on the 90% good. Safety II help you to frame and focus on the positive, it is not about safety II replace safety I but finding the right balance

"So what does Wales have to teach the world about how to deliver better healthcare? The answer is: quite a lot, actually."
