🫀 CardioNugget™ Why might a PFO cause Platypnea-Orthodeoxia Syndrome? A PFO alone usually doesn't cause a shunt because LA pressure > RA pressure keeps it functionally closed. In Platypnea-Orthodeoxia Syndrome (POS), an anatomic change (e.g., ascending aortic aneurysm or atrial septal aneurysm) redirects IVC blood toward the PFO when upright, creating a right-to-left shunt. ➡️ Deoxygenated blood bypasses the lungs → ↓ O₂ saturation (orthodeoxia) + dyspnea (platypnea). Advanced Prostate Cancer Consensus Conference
Germline CDK12 variants in aggressive prostate cancer aacrjournals.org/cancerdiscover… This study identified rare germline CDK12 truncating variants in 5 of 4,535 patients (0.1%) with metastatic #ProstateCancer , challenging the belief that CDK12🧬 alterations are exclusively somatic. All cases demonstrated biallelic CDK12inactivation and the characteristic tandem duplication genomic signature. @LiangRhea Exactly. It’s freedom to speak, not “I must be heard” or immunity from consequences. And we all accept some things cannot be permitted to be said (eg false “Fire!” In a theatre). Even the US 1st amendment is only about limiting what government can do about making relevant laws.

People forget that 'free speech', regardless of social approbation or causing offense, is a US concept. In Australia (Karl) and the UK (Piers), speech follows usual social norms of both responsibility and consequence, as do all wilful human actions.
