@ChloeERodger @macjay72 I think some are viewing this as a 'pothole' and don't understand the nuances of the career pathway, and the payrise is irrelevant for those shouting about that. Don't waste your time trying to explain to others 🫠

I find it wild how many people have told me "but you're just finishing F2 you don't have any real responsibilities you'll be ok" in response to my lack of proper job next week I have a mortgage to pay. And potentially a divorce settlement. But I'm early in my career so it's OK?

@ggkuhnle @lowcarbGP I am still struggling to understand what point he is trying to make here? I'm not a fan of mass produced supermarket bread but that's besides the point.

Biochemistry and physiology are core subjects for everyone studying medicine. That makes it very difficult understand why people like @lowcarbGP choose to completely ignore it and make such misleading statements.
Dr David Unwin@lowcarbGP

Starchy foods like bread digest down into a lot of sugar so @Warburtons your ‘no added sugar’ advertising is very misleading. Today in clinic I saw my third case of poorly controlled Type 2 diabetes lead astray by your packaging PLEASE STOP THIS !! Starch is soon to be sugar! @rcgp

When you’re getting on an airplane do you want the pilot who has years and years of training to fly the plane? Or Do you want the guy with no experience who watched a YouTube video on airplanes who is just asking questions to fly the plane?

@Neuro_Matt Same xp. Had a bully F1 supervisor-gran died- had to travel hours for funeral. He said 'sorry you missed some clinical xp'. No condolences-nothing. Used to tell me I was a crap doc. Also had to do 7x 12.5hr shifts in a row during surgery...BMA&Hosp didn't help...2hr round trip.

In summary: Working in healthcare is wonderful and I genuinely love my job and 99% of the people I work with. But the NHS / HEE do not care about you as individuals, so limit how much of yourself you give them.

1/ Let me tell you a story of the first time I realised the NHS really doesn’t care about us. When I was in F2 I was doing a renal job and had interviews for training posts (ACF). I had three interviews, and one of them (Cambridge) fell in the middle of a run of nights.

One often underappreciated factor in delivering good quality care is having sufficient time to clinically assess patients, identify their concerns and discuss a management plan with them. Compared to many other developed countries, GP consultations in the UK are very short.

This @guardian article is sloppy. ECGs are not "some of the most complex diagnostic procedures carried out by doctors." And ECGs are interpreted by doctors from a range of specialties, not just cardiologists. We need to be realistic about IT in healthcare. theguardian.com/society/2025/j…

@Azeem_Majeed I tell my patients this- it's not 'just' a virus. I wonder where this notion has come from historically.

I sometimes hear the comment “It’s just the flu”. Please remember that influenza like other viral illness can have devastating consequences for people; resulting in serious illness, hospitalisation, long-term complications and sometimes death. Let’s not trivialise any illness.

I just did my first CrossFit foundation class - over COVID I gained significant amounts of weight and that pic on the right was me at my heaviest 99kg in Feb, I restarted the gym on May 15th and have cleared just shy of 20kg in just over 6 months. I promised myself I'd give

Unclear why biological ageing clocks are so popular, esp with construct validity. Many low hanging fruit in way of meaninfgul diagnostics. What is the latest testing you've had? #Longevity #ProactiveCare #BiologicalAge

@Dr_BellaR If admin write a specific time a patient is available, I always endeavour to call them at thay time even though admin will have said no guarantees. No easy answers as we do deal with emergencies and urgent situations in GP also. I've been in your shoes.

‘Many patients have a rising creatinine during diuresis. This is NOT a sign of worsening renal function. Creatinine is measured as a concentration in serum… a creatinine rise *associated with a haematocrit rise* is a sign of reduction in volume & effective decongestion’ 👍🏻
PraveenG@Nephtales

#9 induced diuresis after AKI implies full renal recovery. No, it does not. #10 should be stopped if serum creatinine is increasing, indicating worsening renal function. No, not necessarily. From blog.curem.com.br/wp-content/upl…

Oh what planet is an adult, a trained sniper nonetheless, deliberately shooting a child in the head “self-defence”?! It’s murder. EVERYONE can see it’s murder. The ONLY reason people don’t have a problem with it is because the child in question has brown skin.

@UKGastroDr More time means more alignment of priorities and meeting the patient where they are at if required. Informed decisions in a 10min GP consult is challenging for complex patients with multimorbidity and polypharmacy.

I usually have 30 minutes to see a new patient in clinic, request investigations, prescribe treatment & dictate the letter. I don’t know how GP colleagues do this in 10 minutes. So much skill & expertise required.

@DrJSherrington I do some of this work with a big company but only follow BSSM guidelines with fully informed consent and counselling and I signpost them to see their NHS GP for endocrine input if they're unsure or money is a concern. Some TRT companies are known to be more 'relaxed'.
