This is EXACTLY how a gold command crisis structure should work. This sounds text book and to be commended. HOWEVER the big mistake made here is that Heathrow has responded with a 2019 plan to a 2025 crisis….The 🌍 has changed. Some quick thoughts 🧵

Throat packs retained after surgery can be fatal In 2018 we published national consensus & recommendations about their use Just had paper accepted auditing these 'never events' for 6 years before & after the guidelines were published A 79% reduction was found. Great result 😀
