#HOCM discussion continues - Cath/Surgical Management, led by Dr. Mike Ragosta & Dr. John Kern @UVA - an interesting case @sacsconference ➡️18 y/o male, ex-high school QB, presents with chest pain, SOB. Has a history of anxiety, vaping, marijuana, supplements use. Presented at 13 with possible syncope, + family history of HOCM (mother gene+). Has been refusing genetic testing. Echoes at 13 & 15 were normal. Symptoms now rapidly progressing over weeks. ✅Outcome - Patient with heart block showing no resolution after 3 days. Considering ICD (subcutaneous) as part of the overall treatment plan, specifically a dual-chamber pacer ICD. Question arises: is the need for a pacemaker after septal myectomy indicative of treatment failure? #SACS2023 #AtriumHealth #Cardiology #CardioTwitter

Discussion on cardiac advanced life support unit "When Time Matters" In the ICU @sacsconference @allisonkirkner, NP and Casey Sweet, NP @UVA Great Information on creating a CSU-ALS program. ✅CSU-ALS is an emergency protocol used in place of ACLS for patients within 10 days of sternotomy who undergo cardiac arrest. ✅Trained staff - APPs, residents, intensivists, RNs - perform an emergency resternotomy, aiming to be in the chest within 5 minutes of arrest. ✅Suitable in ICU or stepdown/acute care, depending on staff/acuity. #SACS2023 #AtriumHealth #CardioTwitter #Cardiology

✅Next up @sacsconference Dr. @Carnicelli_Ant of @MUSChealth with the case of - 58y/o woman presenting with chest pain, pre-syncope. Cardiac MRI showed severe asymmetric LV hypertrophy, LVEF 70%, flow acceleration in LVOT. No SAM detected, moderate MR & LA dilation. Intriguingly, spotted patchy hyperenhancement within the hypertrophied septum (3% total burden) ➡️Severe LV dysfunction ➡️Hemorrhage at femoral access site requiring surgery ➡️Heart transplantation workup initiated ➡️OR for extracorporeal LVAD (Rotaflow) ➡️Transaortic septal myectomy ➡️Bacteremia, DIC ➡️RV failure -> RVAD ➡️Withdrawal of care #SACS2023 #AtriumHealth #SangerHeart #CardioTwitter
